替吉奥同步累及野放疗治疗中晚期食管癌疗效评价及副反应分析

摘　　要：目的探讨替吉奥同步累及野放疗治疗中晚期食管癌的近期疗效及不良反应。方法回顾性研究我院2018年1月至2020年10月收治的中晚期食管癌患者,按照治疗方式分为同步放化疗组(替吉奥+同步累及野放疗)和单纯放疗组。两组患者治疗结束后3个月分析两组患者的近期有效率及不良反应。结果同步放化疗组有效率64.5%,高于单纯放疗组的38.2%,差异有统计学意义(P<0.05);同步放化疗组胃肠道反应发生率高于单纯放疗组,差异有统计学意义(P<0.05);骨髓抑制发生率和放射性食管炎发生率两组差异无统计学意义(P>0.05),且两组患者出现的副反应多为1~2级,经对症治疗后均可明显缓解。结论替吉奥同步累及野放疗治疗中晚期食管癌有较好的近期疗效,且副反应可控,安全性较好。Objective To investigate short-term efficacy and adverse reactions of S-1 combined with concurrent involved-field irradiation in the treatment of advanced esophageal cancer.Methods A retrospective study was conducted on patients with advanced esophageal cancer treated in our hospital from January 2018 to October 2020.According to the treatment methods,they were divided into the concurrent chemoradiotherapy group(S-1+concurrent radiotherapy)and the simple radiotherapy group.Three months after the treatment,we analyzed the short-term efficacy rate and adverse reactions of patients in the two groups.Results The efficacy rate of the concurrent chemoradiotherapy group was 64.5%,which was significantly higher than that of the radiotherapy group(38.2%),and the difference was statistically significant(P<0.05).The incidence of gastrointestinal response in the concurrent chemoradiotherapy group was higher than that of the radiotherapy group,and the difference was statistically significant(P<0.05).The incidence of myelosuppression and radioesophagitis was not statistically different in two groups(P>0.05).Most side effects of the two groups were grade one or two,which were relieved after the treatment.Conclusion S-1 combined with concurrent involved-field irradiation in the treatment of advanced esophageal cancer has good short-term efficacy,controllable side effects and safety.
