摘　　要：目的探讨中枢神经系统血管母细胞瘤的临床特征与治疗策略。方法回顾性分析45例中枢神经系统血管母细胞瘤患者的临床资料。患者均经手术切除肿瘤,术后随访观察患者的治疗效果。结果本组患者中,12例患者(26.7%)为实性、28例患者(62.2%)为囊性、5(11.1%)例患者为囊实性血管母细胞瘤。7例患者(15.6%)行术前DSA检查,其中2例患者(4.4%)行术前介入栓塞治疗。肿瘤手术全切的患者37例(82.2%)。术后随访,7例患者(15.6%)出现肿瘤复发,其中6例患者接受放疗。结论手术切除是中枢神经系统血管母细胞瘤的重要治疗手段。部分体积大的实性血管母细胞瘤可先行介入栓塞部分供血动脉后手术切除肿瘤;反复复发的患者可考虑手术联合放疗或抗血管生成靶向药物等综合治疗方案。Objective To investigate the clinical characteristics and treatment strategy of hemangioblastomas of central nervous system.Method The clinical data of 45 patients with hemangioblastoma of central nervous system were analyzed retrospectively.All patients were treated by surgical resection of tumor,and the treatment effect was observed after operation.Results In this group,12 patients(26.7%)were solid,28 patients(62.2%)were cystic,and 5(11.1%)patients hemangioblastoma had both.Preoperative DSA was performed in 7 patients(15.6%),and preoperative interventional embolization was performed in 2 patients(4.4%).Total resection was performed in 37 patients(82.2%).Postoperative follow-up showed that 7 patients(15.6%)had tumor recurrence,of which 6 patients received radiotherapy.Conclusions Surgical resection is an important treatment for hemangioblastoma of central nervous system.For some large solid hemangioblastomas,embolization of some feeding arteries can be performed first and then the tumor can be removed.For recurrent patients,surgery combined with radiotherapy or anti angiogenic targeted drugs should be considered.
