摘 要:目的探讨常规超声和超声造影在泪腺上皮性肿瘤良恶性诊断中的应用价值。方法回顾性病例研究。收集沈阳市第四人民医院2011年4月至2016年3月诊治的52例(52只眼)泪腺上皮性肿瘤患者的临床资料并进行回顾性分析,应用常规超声观察肿瘤的二维及彩色多普勒特点,应用SonoLiver软件分析肿瘤超声造影资料并获得相关曲线及定量参数。结果常规超声联合超声造影诊断良性肿瘤、恶性肿瘤与病理结果的符合率分别为66.7%(20/30)、68.2%(15/22)。超声造影定量参数:峰值强度(IMAX)恶性组高于良性组,上升时间(RT)、平均渡越时间(mTT)恶性组低于良性组,差异具有统计学意义(P<0.05);达峰时间(TTP)两组间差异无统计学意义(P>0.05)。结论应用常规超声和超声造影检查可为泪腺上皮性肿瘤良恶性诊断提供可靠的量化依据,具有较高的临床应用价值。Objective To investigate the value of conventional ultrasonography and contrast-enhanced ultrasonography in the diagnosis of benign and malignant lacrimal gland epithelial tumors.Methods The clinical data of 52 patients with lacrimal gland epithelial tumor at the Fourth People's Hospital of Shenyang from April 2011 to March 2016 were collected and analyzed.The two-dimensional and the color doppler characteristics of the tumor were observed by the conventional ultrasound.The tumors’contrast-enhanced ultrasound data was analyzed by SonoLiver and the relation curves and quantitative parameters were obtained.Results The consistency rate of conventional ultrasonography and contrast-enhanced ultrasonography in the diagnosis of benign and malignant tumors with the pathological results was 66.7%(20/30)and 68.2%(15/22)respectively.Regarding the quantitative parameters of contrast-enhanced ultrasound,the peak intensity(IMAX)was higher in the malignant group than the benign group,while the rise time(RT)and mean transit time(mTT)was lower in the malignant group than the benign group,and the differences were statistically significant(P<0.05).There was no significant difference in the time to peak(TTP)between the two groups(P>0.05).Conclusions Conventional ultrasonography and contrast-enhanced ultrasonography can provide a reliable quantitative basis for the diagnosis of benign and malignant lacrimal gland epithelial tumors,which has a high clinical application value.
