摘　　要：目的评价多参数MRI在膀胱癌二次电切术前诊断肿瘤残留的价值。方法回顾性分析河南省肿瘤医院泌尿外科自2014年6月至2020年5月收治的29例行膀胱癌二次电切术患者的资料。所有患者术前1周内行盆腔MRI检查,并与术后病理结果对比,评估MRI在诊断膀胱癌病灶残留方面的敏感度、特异度及准确度。结果本组共29例患者,平均年龄(63.38±12.42)岁,其中男23例,女6例。术后病理证实膀胱癌病灶残留者11例,无残留者18例。采用T2WI、T2WI+DWI诊断膀胱癌病灶残留的敏感度分别为90.91%和45.45%、特异度分别为38.89%和83.33%、准确度分别为58.62%和68.97%。结论多参数MRI在膀胱癌二次电切术前诊断膀胱癌病灶残留方面可能有一定的应用价值。Objective To evaluate the performance of mpMRI for the local detecting of residual disease for bladder cancer following transurethral resection of bladder tumor.Methods We retrospectively studied 29 patients with pathology-proven bladder cancer who underwent re-TURBT between June 2014 and May 2020,and all patients were from department of urology in Henan Cancer Hospital.All patients underwent MRI within one week before re-TURBT.Sensitivity,specificity and accuracy were calculated for the presence of residual disease by TWI only and TWI+DWI compared to pathological results from the re-TURBT.Results Of 29 patients,37.93%(11/29)were found with the presence of residual disease depending on pathology from re-TURBT.TWI only and TWI+DWI of MRI for readers were sensitivity 90.91%,45.45%;specificity 38.89%,83.33%;accuracy 58.62%,68.97%,respectively.Conclusions MpMRI may have some value at detecting the presence of residual disease for bladder cancer before re-TURBT.
