腹膜后Castleman病7例临床分析

摘　　要：目的探讨腹膜后Castleman病(CD)的诊治方式及进展。方法回顾性分析苏州大学附属第一医院泌尿外科2010年1月至2020年1月收治的7例腹膜后CD患者的临床资料。患者平均年龄51岁,术前行B超、CT或MRI检查,均提示腹膜后肿物,治疗上予以手术切除肿大淋巴结或取活检,并配合内科综合治疗。结果7例患者肿物切除手术均顺利,术后病理提示局灶型CD(UCD)患者6例,全部为透明血管型;多中心型CD(MCD)患者1例,为浆细胞型。UCD患者术后定期随访,均未作进一步治疗,MCD患者术后规律化疗。7例患者均预后良好。结论腹膜后CD常为UCD病变,多无明显临床症状,最终诊断依靠病理。UCD患者手术切除疗效好,术后可长期生存,MCD患者术后配合规范的内科治疗,亦可延长患者生存期。Objective To explore the diagnosis,treatment methods and progress of retroperitoneal Castleman's disease(CD).Methods The clinical data of 7 patients with retroperitoneal CD admitted to the Department of Urology,First Affiliated Hospital of Soochow University from January 2010 to January 2020 were retrospectively analyzed.The median age of the patients was 51 years old.Preoperative B-ultrasound,CT or MRI examinations showed retroperitoneal masses.In treatment,enlarged lymph nodes were surgically removed or biopsy was taken,and comprehensive medical treatment was performed.Results Seven cases of tumor resection were successful.Postoperative pathology showed 6 cases of focal type,and all of which were hyaline vascular type.One case of multicentric type was plasma cell type.The focal type patients were followed up regularly without further treatment,while the multicentric type patient received regular postoperative chemotherapy.All patients have a good prognosis.Conclusions Retroperitoneal CD is often a focal lesion with no obvious clinical symptoms.The final diagnosis depends on pathology.For focal CD,surgical resection has a good effect and prognosis.For multicentric CD,surgery combined with standard medical treatment can also prolong the survival.
