摘 要:目的比较育龄期与绝经后女性乳腺超声类型的差异,探讨乳腺结构与乳腺病变的相关性。方法收集了16 357例育龄期组与绝经后组乳腺超声声像图及757例(860个病灶)手术病例的病理检查结果。乳腺分类的方法分为两步,第一步根据腺体厚度占比及成分不同分为腺体型、腺纤维型和脂肪型三类;第二步将腺纤维型进一步分为腺纤维1型和腺纤维2型。比较分析两组女性的乳腺超声类型与病变的关系。结果育龄期组与绝经后组乳腺类型的分布呈相反的变化趋势;两组在两步分型法中乳腺超声类型占比差异均有明显统计学意义。恶性病例中两组均为脂肪型的标准化发病率最高,腺体型中尚未发现有恶性病变。结论育龄期与绝经后女性乳腺类型的分布具有明显差异,但发生乳腺恶性病变的背景类型相似;掌握乳腺类型的声像图表现,对监测乳腺恶性病变的发生具有重要的临床意义。Objective To compare the differences of breast ultrasound types between childbearing age and postmenopausal women, and to explore the correlation between breast gland structure and breast lesions. Methods The breast ultrasound images of 16 357 childbearing age and postmenopausal women as well as the pathological examination results of 757 surgical cases(860 focus) were collected. The classification of breast types is divided into two steps. The first step is to divide the breast parenchyma into three types according to the gland thickness ratio and compositiofeatures, namely glandular type, fibroglandular type, and fat type;the second step is to further divide fibroglandular type into fibroglandular type 1 and fibroglandular type 2. The correlation between breast types and lesions was analyzed between the two groups. Results The distribution of mammary gland types during childbearing age and postmenopausal age showed an opposite trend. There is a statistical difference in the proportions of the breast types between the two groups. Among the malignant cases, both the two groups had the highest standardized incidence in fat type. No malignant lesions were found in the glandular type. Conclusions The distribution of breast types in childbearing age and postmenopausal women is significantly different, but the histopathological background types of breast malignant lesions are similar;it has an important clinical significance to master the ultrasonographic features of breast types for monitoring the occurrence of breast malignant lesions.
