摘 要:目的探讨CT灌注成像在肝癌靶向治疗疗效评价中的应用价值。方法选择2018年1月至2020年1月医院收治的68例肝癌患者,分别在患者接受肝癌靶向治疗前和治疗1个月后进行CT灌注成像检测,比较治疗前和治疗1个月后患者的门静脉灌注量(PVP)、肝动脉灌注指数(HPI)、肝动脉灌注量(HAP),随访6个月后比较治疗有效和无效患者的上述指标。结果治疗1个月后,患者的PVP、HPI、HAP均低于治疗前,差异有统计学意义(P<0.05)。随访结果显示,68例患者中,治疗有效42例,治疗无效26例。治疗前,两组PVP、HPI、HAP比较,差异无统计学意义(P>0.05);无效组治疗前和治疗1个月后的PVP、HPI、HAP比较,差异无统计学意义(P>0.05);有效组治疗1个月后的PVP、HPI、HAP均低于同组治疗前和无效组治疗1个月后,差异均有统计学意义(P<0.05)。结论CT灌注成像能够有效反映肝癌患者靶向治疗前后肝脏内血流动力学的变化情况,为临床疗效评价提供依据。Objective The application value of CT perfusion imaging in evaluating the therapeutic effects of liver cancer targeted therapy were explored.Methods A total of 68 patients with liver cancer admitted to the hospital from January 2018 to January 2020 were selected.CT perfusion imaging was performed before and 1 month after liver cancer targeted therapy,respectively.Portal vein perfusion(PVP),hepatic artery perfusion index(HPI)and hepatic artery perfusion(HAP)were compared before and 1 month after treatment.After 6 months of follow-up,the mentioned indicators were compared between patients who were treated effectively and those who were not.Results After one month of treatment,the PVP,HPI and HAP of all patients were significantly reduced compared with before treatment(P<0.05).The follow-up results showed that among the 68 patients,42 patients were treated effectively and the other 26 patients were not.Before treatment,when the PVP,HPI and HAP indicators were compared between the two groups,the difference was not statistically significant(P>0.05);When the PVP,HPI and HAP indicators of the patients who were not treated effectively were compared before and 1 month after treatment,the difference was not statistically significant(P>0.05);The PVP,HPI and HAP indicators of the patients who were treated effectively were lower than those before treatment of themselves and the patients who were not treated effectively after one month treatment,and the differences were both statistically significant(P<0.05).Conclusion CT perfusion imaging can effectively reflect the changes of hemodynamics in the liver of patients with liver cancer before and after targeted therapy,and can provide a basis for clinical efficacy evaluation.
