玻璃体切除联合内界膜剥离术治疗外伤性黄斑裂孔患者的前瞻性研究

摘　　要：目的分析玻璃体切除联合内界膜剥离术治疗外伤性黄斑裂孔(TMH)患者的效果。方法选取2018年2月至2020年8月杞县德明杨氏眼科医院收治的58例TMH患者,依照随机数表法分为两组,各29例。对照组接受玻璃体切除术治疗,观察组接受玻璃体切除联合内界膜剥离术治疗,比较两组术前及术后3、6个月视力水平,术前及术后1、2、3、6个月黄斑裂孔直径,以及术后1、2、3、6个月视力提高情况及黄斑裂孔闭合情况。结果术后3、6个月,两组视力水平均提升,且观察组较对照组高(P<0.05);术后1、2、3、6个月,两组黄斑裂孔直径均缩小,且观察组小于对照组(P<0.05);两组术后1、2、3个月视力水平对比,差异无统计学意义(P>0.05),观察组术后6个月视力水平较对照组高(P<0.05);两组术后1、2、3、6个月黄斑裂孔闭合率对比,差异无统计学意义(P>0.05)。结论玻璃体切除联合内界膜剥离术治疗TMH,能提高患者视力水平,促进黄斑裂孔愈合。Objective To analyze the effect of vitrectomy combined with internal limiting membrane dissection in the treatment of traumatic macular hole(TMH).Methods A total of 58 TMH patients in Qi County Deming Yang’s Eye Hospital from February 2018 to August 2020 were selected and divided into two groups according to the random number table method,with 29 cases in each group.The control group received vitrectomy,and the observation group received vitrectomy combined with internal limiting membrane dissection.The visual acuity levels of the two groups were compared before operation,3 months after operation,and 6 months after operation.The diameter of the macular hole at 1,3,6 months,as well as the improvement in visual acuity and closure of the macular hole at 1,2,3,6 months after surgery.Results The visual acuity levels of the two groups were improved at 3 months and 6 months after the operation,and the observation group was higher than the control group(P<0.05).After 1,2,3,6 months after operation,the diameters of the macular holes in the two groups were reduced,and the macular holes in observation group were smaller than those in control group(P<0.05).There was no statistical difference in visual acuity between the two groups after 1,2,3 months after surgery(P>0.05),while the visual acuity in observation group was higher than that in control group after 6 months after operation(P<0.05).There was no statistical difference in comparison of macular hole closure rates between the two groups after 1,2,3,6 months after operation(P>0.05).Conclusion Vitrectomy combined with internal limiting membrane dissection for TMH can improve the visual acuity of patients and promote the healing of the macular hole.
