摘　　要：目的比较电视胸腔镜(VATS)下肺段切除术与肺叶切除术治疗Ⅰ期非小细胞肺癌(NSCLC)的远期疗效,为临床治疗提供循证证据。方法通过计算机检索PubMed、EMBASE、web of science、the Cochrane Library、CNKI、CBM、VIP、WanFang等数据库,检索时限为建库至2020年3月13日,搜集VATS下肺段切除与肺叶切除治疗Ⅰ期NSCLC的临床对照研究,对其进行资料提取和质量评价,采用STATA15.1软件进行Meta分析。结果共纳入12篇文献,共2515例患者,其中肺段切除术783例(肺段切除组),肺叶切除术1732例(肺叶切除组)。Meta分析结果显示:肺段切除组与肺叶切除组OS[HR=1.02,95%CI(0.77,1.34),P=0.905]、DFS[HR=1.06,95%CI(0.85,1.31),P=0.624]比较,差异无统计学意义(P>0.05)。结论VATS下,肺段切除与肺叶切除治疗Ⅰ期NSCLC其远期疗效相当,VATS下肺段切除术是一种可供临床选择的早期NSCLC根治术式。Objective To compare the long-term effects of video-assisted thoracoscopic surgery(VATS)segmentectomy and lobectomy on stageⅠnon-small cell lung cancer(NSCLC),and to provide evidence-based evidence for the clinical treatment.Methods The PubMed,EMBASE,Web of Science,Cochrane Library,CNKI,CBM,VIP and WanFang were searched from inception through March 13,2020 for the controlled clinical trials relevant to the treatment of stageⅠNSCLC with VATS segmentectomy and lobectomy.After data extraction and quality evaluation,meta-analysis was performed using STATA15.1 software.Results A total of 12 studies were included in this analysis.Among the 2515 patients,783 underwent VATS segmentectomy and 1732 underwent VATS lobectomy.Meta-analysis showed that there were no significant differences in OS(HR=1.02,95%CI(0.77,1.34),P=0.905)and DFS(HR=1.06,95%CI(0.85,1.31),P=0.624)between VATS segmentectomy group and VATS lobectomy group(P>0.05).Conclusion VATS segmentectomy has the same long-term effect as VATS lobectomy on stageⅠNSCLC.Therefore,VATS segmentectomy is an alternative radical surgery for early NSCLC.
