玻璃体切除空气填充术治疗视网膜脱离的应用效果

摘　　要：目的探究视网膜脱离患者应用玻璃体切除空气填充术治疗的效果。方法60例(60眼)视网膜脱离患者为研究对象,均行玻璃体切除空气填充术治疗。术后随访6~12个月,比较患者术前、术后1周、术后3个月、末次随访时的眼压,气体吸收及视网膜复位情况,并发症发生情况。结果60例(60眼)患者术后1周、术后3个月、末次随访时的眼压分别为(16.5±9.6)、(15.2±3.2)、(15.7±2.5)mm Hg(1 mm Hg=0.133 kPa),均高于术前的(12.6±3.6)mm Hg,差异均具有统计意义(t=2.946、4.181、5.479,P<0.05)。患者玻璃体腔气泡平均吸收时间为(10.25±2.3)d。所有患者术后1周视网膜均复位,复位率为100.0%。有3眼于术后30 d出现斑孔性视网膜脱离,对患者进行硅油填充并联合内界膜剥除治疗后,患眼视网膜复位情况均达到良好,到末次随访时,患眼视网膜复位率为100.0%。术中4眼出现晶体浑浊,行玻璃体切割手术(PPV)联合白内障超声乳化人工晶体植入术治疗。无一例患者发生眼内炎或脉络膜上腔出血等并发症。结论视网膜脱离患者使用玻璃体切除空气填充术治疗能够起到良好的治疗效果,患者术后并发症发生率低、视网膜复位率高、视力恢复效果良好、恢复时间较短,具有临床应用价值。Objective To investigate the effect of vitrectomy and air tamponade on the treatment of retinal detachment.Methods 60 patients(60 eyes)with retinal detachment were the subjects of the study,all underwent vitrectomy and air tamponade.All patients were followed up for 6-12 months after surgery,and the intraocular pressure,gas absorption,retinal reattachment,and complications before surgery,1 week after surgery,3 months after surgery,and at the last follow-up were compared.Results The intraocular pressures of 60 patients(60 eyes)were(16.5±9.6),(15.2±3.2),(15.7±2.5)mm Hg(1 mm Hg=0.133 kPa)at 1 week after surgery,3 months after surgery,and at the last follow-up,which were higher than(12.6±3.6)mm Hg before surgery,and the differences were statistically significant(t=2.946,4.181,5.479,P<0.05).The average absorption time of bubbles in the vitreous cavity of the patient was(10.25±2.3)d.All patients had retina reattachment 1 week after operation,and the reattachment rate was 100.0%.3 eyes developed patchy retinal detachment at 30 d after surgery.After treatment with silicone oil filling and combined with inner boundary membrane peeling,the retinal reattachment of the affected eyes reached good conditions,and the retinal reattachment rate was 100.0%at the last follow-up.During the operation,4 eyes developed lens opacity,and were treated with pars plana vitrectomy(PPV)combined with phacoemulsification and intraocular lens implantation.None of the patients experienced complications such as endophthalmitis or suprachoroidal hemorrhage.Conclusion Vitrectomy and air tamponade has a good therapeutic effect on patients with retinal detachment,with low postoperative complication rate,high retinal reattachment rate,good visual recovery effect and short recovery time,which has clinical application value.
