摘 要:目的:探究早期胃癌(EGC)的形态学分型、病理组织学分型与p53、c-erbB-2表达强度的关系。方法:对2016年3月至2018年3月于本院经白光内镜、窄带成像放大内镜、CT检查及病理活检或者手术确诊的68例EGC患者进行形态学分型及病理组织学分型,免疫组化检测肿瘤组织p53、c-erbB-2蛋白表达,分析p53、c-erbB-2表达与胃癌形态学分型及病理组织学分型的相关性。结果:68例EGC患者中,Ⅰ型14例,占20.59%;Ⅱa型8例,占11.76%;Ⅱb型6例,占8.82%;Ⅱc型22例,占32.35%;Ⅲ型18例,占26.47%。单发66例(97.06%),多发2例(2.94%),共71个病灶;其中管状腺癌25例(36.76%),低分化腺癌22例(32.35%),混合型癌12例(17.65%)、印戒细胞癌7例(10.29%)、黏液腺癌2例(2.94%)。p53蛋白阳性表达35例(51.47%),阴性表达33例(48.53%);c-erbB-2蛋白阳性表达13例(19.12%),阴性表达55例(80.88%)。Spearman相关性分析显示,内窥镜形态学分型与p53表达强度呈正相关(r=0.397,P=0.001),而与c-erbB-2表达强度无相关性(r=-0.108,P=0.191)。病理组织学分型与p53表达强度呈正相关(r=0.366,P=0.032),病理组织学分型与c-erbB-2表达强度呈正相关(r=0.345,P=0.037)。结论:EGC病理组织学分型与p53、c-erbB-2表达强度相关,形态学分型与p53表达强度相关。Objective:To explore the morphological and histopathological types of early gastric cancer(EGC)and their relationships with the expression intensities of p53 and c-erbB-2.Methods:From March 2016 to March 2018,68 patients with early gastric cancer confirmed by serology,narrow-band imaging magnifying endoscopy and computed tomography,pathological biopsy or surgery were selected.Immunohistochemistry was used to detect the expressions of p53 and c-erbB-2,and the correlations between the expressions of p53 and c-erbB-2 and the morphological and histopathological types of EGC patients were analyzed.Results:Among 68 patients with EGC,14 were typeⅠ(20.59%),8 were typeⅡa(11.76%),6 were typeⅡb(8.82%),22 were typeⅡc(32.35%)and 18 were typeⅢ(26.47%).66 cases(97.06%)were single lesion,2 cases(2.94%)were multiple lesion,and there were 71 lesions,including 25 cases of tubular adenocarcinoma(36.76%),22 cases of poorly differentiated adenocarcinoma(32.35%),12 cases of mixed cancer(17.65%),7 cases of signet ring cell carcinoma(10.29%)and 2 cases of mucinous adenocarcinoma(2.94%).There were 35 cases(51.47%)with positive expression of p53 protein and 33 cases(48.53%)with negative expression,there were 13 cases(19.12%)with positive of c-erbB-2 protein and 55 cases(80.88%)with negative expression.Spearman correlation analysis showed that endoscopic morphological classification was positively correlated with p53 expression intensity(r=0.397,P=0.001),but not with c-erbB-2 expression intensity(r=-0.108,P=0.191).Pathological histological classification was positively correlated with p53 expression intensity(r=0.366,P=0.032),while positively correlated with c-erbB-2 expression intensity(r=0.345,P=0.037).Conclusions:The pathological classification of EGC patients was correlated with the expression intensities of p53 and c-erbB-2,and the morphological typing was correlated with the expression intensities of p53.
