癫痫耐药患者血清和脑脊液中P-糖蛋白、转化生长因子-β1及谷胱甘肽S转移酶的表达  被引量:6

摘  要:目的观察难治性癫痫患者血清和脑脊液中P-糖蛋白(P-glycoprotein,P-gp)、转化生长因子(transforming growth factor,TGF)β1及谷胱甘肽S转移酶(glutathione S-transferase,GST)的表达。方法选取2018年2月~2020年11月期间解放军联勤保障部队第983医院规范治疗2年以上的癫痫耐药患者79例作为耐药组,同期接受抗癫痫治疗控制良好的患者60例作为敏感组。比较敏感组和耐药组脑脊液、血清中P-gp、GST、TGF-β1水平差异,分别计算同组患者脑脊液和血清中P-gp、GST、TGF-β1的相关性。结果耐药组脑脊液中P-gp、GST、TGF-β1水平均高于敏感组,两组间差异均有统计学意义(P<0.05)。耐药组血清中P-gp、GST水平高于敏感组,两组间差异均有统计学意义(P<0.05);TGF-β1水平差异无统计学意义(P>0.05)。耐药组和敏感组的血清和脑脊液中P-gp和GST表达正相关,耐药组P-gp的相关系数为0.873,GST相关系数为0.891。耐药组和敏感组脑脊液和血清TGF-β1表达无相关性。结论癫痫耐药患者脑脊液中P-gp、GST、TGF-β1水平升高,外周血P-gp、GST水平升高,监测外周血的P-gp、GST变化可能为癫痫药物治疗方案制定提供参考。Objective To observe the expression of P-glycoprotein(P-gp),transforming growth factor(TGF)β1 and glutathione S-transferase(GST)in serum and cerebrospinal fluid of patients with intractable epilepsy.Methods From February 2018 to November 2020,79 patients with epilepsy drug resistance who had been treated for more than 2 years were selected as the drug resistance group,and 60 patients with well controlled epilepsy were selected as the sensitive group.The levels of P-gp,GST and TGF-β1 in cerebrospinal fluid and serum were compared between the sensitive group and the drug-resistant group.The correlation of P-gp,GST and TGF-β1 in cerebrospinal fluid and serum was calculated in the same group.Results The levels of P-gp,GST and TGF-β1 in cerebrospinal fluid of drug-resistant group were higher than those of sensitive group,and the differences of the three indexes between the two groups were statistically significant(P<0.05).The levels of P-gp and GST in the drug-resistant group were higher than those in the sensitive group(P<0.05).There was no significant difference in TGF-β1 level between drug-resistant group and sensitive group(P>0.05).There was a positive correlation between P-gp and GST expression in serum and CSF of drug-resistant group and sensitive group.The correlation coefficient of P-gp and GST in drug-resistant group was 0.873 and 0.891,respectively.There was no correlation between the expression of TGF-β1 in CSF and serum in drug-resistant group and sensitive group.Conclusion The levels of P-gp,GST,TGF-β1 in CSF and P-gp,GST in peripheral blood of patients with epilepsy drug resistance are increased.Monitoring the changes of P-gp and GST in peripheral blood may provide reference for the development of epilepsy drug treatment plan.
