摘 要:目的检测肝细胞癌(HCC)患者循环血样中外泌体LncRNA HOTAIR和miR-122表达情况与患者临床病理特征以及血清肿瘤标志物的关系。方法收集2019年1月-2020年1月在本院确诊的97例HCC患者,并选取50例非恶性病变患者和50例健康人的血清样本分别作为良性对照和正常对照组。提取受试者血清外泌体,采用QPCR法检测外泌体LncRNA HOTAIR和miR-122相对表达量,并检测血清甲胎蛋白(AFP)、异常凝血酶原Ⅱ(PIVKA-Ⅱ)水平。结果与健康对照组和良性对照组比较,HCC组患者外泌体LncRNA HOTAIR相对表达量升高,同时miR-122相对表达量相应降低(P<0.05);且HCC患者血清外泌体LncRNA HOTAIR和miR-122相对表达量呈负相关性,r=-0.740,P<0.001。此外,HCC患者血清外泌体LncRNA HOTAIR和miR-122相对表达量与血清AFP、PIVKA-Ⅱ有关(P<0.05)。经ROC曲线分析,血清外泌体LncRNA HOTAIR和miR-122相对表达量对于HCC以及HCC患者肝外转移诊断的曲线下面积分别为0.773(95%CI:0.649-0.876)、0.818(95%CI:0.674-0.923)和0.878(95%CI:0.812-0.944)、0.765(95%CI:0.671-0.859),且对于HCC肝外转移的诊断效能则高于血清AFP和PIVKA-Ⅱ(P<0.05)。结论 HCC患者循环血外泌体LncRNA HOTAIR表达升高,同时miR-122表达降低;这与患者血清中AFP和PIVKA-Ⅱ水平及肝外转移风险有关。Objective To detect the expression of circulating exosomal LncRNA HOTAIR and miR-122in hepatocellular carcinoma(HCC)patients and their relationships with conventional serum tumor markers.Methods A total of 97newly diagnosed HCC patients were collected from Wuhan Hanyang Hospital from January 2019to January 2020,while serum samples from 50patients with non-malignant lesions and 50healthy subjects were taken as benign control group and normal control group.The relative expression of LncRNA HOTAIR and miR-122was detected by QPCR,and the serum levels of alpha-fetoprotein(AFP)and protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ)were detected.Results Compared with the healthy control group or benign control group,the relative expression of exosomal LncRNA HOTAIR increased,while miR-122decreased correspondingly(P<0.05).The relative expression of exosomal LncRNA HOTAIR and miR-122in serum of HCC patients was negatively correlated,r=-0.740,P<0.001.In addition,exosomal LncRNA HOTAIR and miR-122levels in HCC patients were both correlated with the serum AFP,PIVKA-Ⅱlevels(P<0.05).By ROC curve analysis,the area below the curve of exosomal LncRNA HOTAIR level and miR-122levels for the diagnosis of HCC and extrahepatic metastasis were respectively 0.773(95%CI:0.649-0.876),0.818(95%CI:0.674-0.923)and 0.878(95%CI:0.812-0.944),0.765(95%CI:0.671-0.859),and the diagnostic efficiency of extrahepatic metastasis of HCC was higher than that of serum AFP and PIVKA-Ⅱ(P<0.05).Conclusion The circulating exosomal LncRNA HOTAIR level in HCC patients is increased,while exosomal miR-122level is decreased correspondingly,which would be both related to AFP and PIVKA-Ⅱlevels and extrahepatic metastasis.
