摘　　要：目的探讨彩超声像图色阶在鉴别小肝癌癌结节和肝硬化假小叶结节中的价值。方法回顾性分析2015年5月至2020年6月重庆市第五人民医院收治的14例肝硬化患者(75个结节性病灶)作为研究对象。选取病灶声像图进行软件分析,计算癌结节和假小叶结节的图像色阶值,并评估色阶值在诊断结节性质中的灵敏度及特异度。结果癌结节的色阶值小于假小叶结节,差异具有统计学意义(P<0.05);两种结节的直径比较,差异无统计学意义(P>0.05)。ROC曲线分析显示,色阶值诊断假小叶结节的曲线下面积为0.717(P=0.005),色阶值为1.51时,判断假小叶结节的灵敏度为63.2%,特异度为82.1%。结论彩超声像图色阶在鉴别小肝癌癌结节和肝硬化假小叶结节中的价值显著,两种结节的色阶值存在差异,可为今后机器智能识别结节性质提供参考。Objective To explore the value of color scale of the color ultrasound image in differentiating small liver cancer nodules and cirrhotic pseudolobular nodules.Methods A total of 14 patients with cirrhosis(75 nodular lesions)admitted in the fifth people's hospital of Chongqing from May 2015 to June 2020 were retrospectively analyzed.The sonograms of lesions were selected for software analysis,the image color scale value of cancer nodules and cirrhotic pseudolobular nodules were calculated,and the sensitivity and specificity of color scale value in diagnosing the nature of nodules were evaluated.Results The color scale value of cancer nodules was less than that of cirrhotic pseudolobular nodules,and the difference was statistically significant(P<0.05);there was no significant difference in the diameter of the two nodules(P>0.05).ROC curve analysis showed that the area under the curve of color scale value in the diagnosis of cirrhotic pseudolobular nodules was 0.717(P=0.005),and when the color scale value was 1.51,the sensitivity and specificity of judging cirrhotic pseudolobular nodules were 63.2%and 82.1%.Conclusion The color scale of the color ultrasound image is of great value in differentiating small liver cancer nodules and cirrhotic pseudolobular nodules,and the color scale values of the two nodules are different,which can provide a reference for the intelligent recognition of the nature of the nodules in the future.
