摘　　要：目的:探讨苦参凝胶改善宫颈病变患者行宫颈环形电切术(LEEP)的治疗效果。方法:选择经宫颈细胞学及HPV-DNA-阴道镜下宫颈活检三阶梯方法确诊的宫颈上皮内瘤变(CIN)Ⅱ、Ⅲ级患者230例,分为观察组(LEEP术+苦参凝胶治疗)和对照组(LEEP术治疗),监测并比较两组术后阴道流血(排液)量及持续时间、创面愈合时间、宫颈管粘连、HPV转归等指标。结果:观察组术后阴道流血量较对照组明显减少(P<0.05),阴道排液时间、宫颈创面愈合时间均较对照组明显缩短(P<0.05),观察组宫颈粘连发生率(1.69%vs 7.14%)明显较低(P<0.05),HPV16、18转阴率(89.47%vs 70%)明显较高(P<0.05)。结论:LEEP术治疗后联合应用苦参凝胶,可明显减少LEEP术后阴道出血及减少排液的时间,缩短创面愈合时间,减少宫颈粘连,提高HPV16、18转阴且无副作用。Objective:To evaluate the efficacy of Sophora flavescens gel in treatment of CIN after loop electrosurgical excision procedure(LEEP).Methods:230 patients who underwent LEEP were selected in this study,all cases were diagnosed as cervical intraepithelial neoplasia(CINⅡ、Ⅲ)by pathologist before surgery.According to the simple randomization grouping method,they were divided into two groups.The observation group(118 cases)was given Sophora flavescens gel after LEEP;the control group(112 cases)was given LEEP only.Vaginal bleeding,cervical healing,cervical adhesion,HPV variety were observed during the postoperative follow-up.Results:The observation group had less vaginal blood,drainage,had lower cervical adhesion(1.69%vs 7.14%),and more positive HR-HPV patients(89.47%vs 70%)converse to negative in observation group(P<0.05).Conclusion:The using of Sophora flavescens gel after LEEP in(CINⅡ、Ⅲ)patients can reduce the postoperative bleeding and drainage,shorten the cervical healing time,reduce the incidence of cervical adhesion,have better clinical outcome in HPV16,18 curing.
