摘 要:目的:从护士视角下剖析新生儿重症监护室(NICU)低母乳喂养率的深层次原因,构建归因模型及干预路径。方法:运用扎根理论的质性研究方法对三级甲等医院NICU的21名护士进行观察和访谈,在此基础上构建归因模型及干预路径。结果:确定了知识⁃选择、心理⁃偏向、资源⁃工具、制度⁃冲突4个主范畴,通过4种诱因匹配机制,构建了护士视角下NICU低母乳喂养率的归因模型及干预路径。结论:知识⁃选择是NICU低母乳喂养率的根本诱因,心理⁃偏向是直接诱因,资源⁃工具是外在诱因,制度⁃冲突是宏观诱因。提高NICU母乳喂养率需要NICU护士和患儿父亲、母亲的共同参与。Objective:The deep reasons for low breastfeeding rate in NICU were analyzed from the perspective of nurses,and the attribu⁃tion model and intervention path were constructed.Methods:The qualitative research method of grounded theory was used to observe and interview 21 nurses in a comprehensive tertiary grade A hospital.On this basis,the attribution model and intervention path were constructed.Results:Four main categories were determined,which were knowledge-choice,psychology⁃bias,resources⁃tools,and institutions⁃conflict.Through these four incentive matching mechanisms,the attribution model and intervention path of low NICU breast⁃feeding rate from the perspective of nurses were constructed.Conclusion:For the low NICU breastfeeding rate,knowledge⁃selection is the underlying cause,psychosocial bias is the direct cause,resources⁃tools is the external cause,and institutional⁃conflict is the macro trigger factor.Increasing breastfeeding rate in the NICU needs the joint participation of the NICU nurse and the parents of the children.
