五苓散在神经重症并发水液代谢障碍治疗中的应用进展

摘　　要：近年来,随着神经重症学科的不断发展,临床中发现神经重症的患者常并发有水液代谢障碍,对患者的预后有着较大的影响。长期的西药治疗容易引起肾功能损害、离子紊乱、反跳现象等不良反应,存在一定的局限性。中医注重整体调节,标本兼治,疗效持久,辅助治疗神经重症中并发水液代谢障碍成为热点之一。五苓散最早出自于汉代张仲景的《伤寒杂病论》中,可用于调节机体水液代谢障碍,为治疗水湿内停的经典方剂。因五苓散利水、利尿作用显著,被称为中医的“利尿剂”,现代研究认为五苓散是一种潜在的广泛用于与水液代谢障碍有关疾病的抗水肿药物,因其多机制、多靶点的治疗特点在改善脑水肿临床症状和实验室指标方面有明显的优势,具有安全有效、取材方便、价格低廉等特点,但五苓散在神经重症并发水液代谢障碍方面的应用和研究并不多见。现就五苓散在神经重症中并发水液代谢障碍的应用进展做一综述。In recent years,with the continuous development of neuro-critical diseases,it has been found that patients with neuro-critical diseases often have complicated water-liquid metabolism disorders,which has a greater impact on the prognosis of patients.Long-term western medicine treatment can easily cause adverse reactions such as renal damage,ion disturbance,rebound phenomenon,etc.,which cause certain limitations.Traditional Chinese medicine pays attention to overall regulation,treats both symptoms and root causes,has low side effects,and has a long-lasting effect.It has become one of the hotspots in adjuvant treatment of neurosurgery complicated with water and fluid metabolism.Wulingsan was first produced in“treatise on febrile and miscellaneous diseases”written by ZHANG Zhongjing in the Han dynasty.It can be used to regulate the body’s water and fluid metabolism disorders,and is a classic prescription for the treatment of hydration.Due to its remarkable diuretic and diuretic effects,Wulingsan is known as the“diuretic”of Chinese medicine.Modern research believes that Wulingsan is a potential anti-edema drug widely used in diseases related to hydrometabolic disorders due to its multiple mechanisms,and it have obvious advantages in improving the clinical symptoms and laboratory indicators of cerebral edema.It has the characteristics of safety and effectiveness,convenient acquisition of materials,and low price.However,the application of Wulingsan in severe neurosis complicated with hydrometabolic disorders research is rare.Therefore,this article summarizes the application progress of Wulingsan in severe neurosis complicated with hydrometabolic disorders.
