摘 要:目的探讨服用与未服用抗精神病药精神分裂症(schizophrenia,SZ)患者基于体素脑镜像同伦功能连接(voxel-mirrored homotopic connectivity,VMHC)特点及其与精神症状之间的关系。方法纳入60例SZ患者(根据是否服用抗精神病药物分为服药患者组40例和未服药患者组20例)和60名健康对照者(对照组),进行静息态功能磁共振检查,计算VMHC值,采用阳性和阴性症状量表(positive and negative syndrome scale,PANSS)评估患者的精神症状。结果3组之间VMHC值差异有统计学意义的脑区位于眶额叶、苍白球、枕中回、中央前回和中央后回(P<0.05,GRF校正)。两两比较显示,与对照组比较,服药患者组枕中回、中央前回和中央后回VMHC值降低(P<0.05),未服药患者组相比于对照组双侧苍白球、枕中回、中央前回和中央后回VMHC值降低(P<0.05),服药患者组较未服药患者组VMHC值降低的脑区是双侧眶额叶(P<0.05)。服药患者组双侧眶额叶VMHC值与阳性症状分呈负相关(r=-0.45,P=0.01)。结论SZ患者静息状态下感觉运动网络和视觉网络大脑半球间功能连接减弱。服用与未服用抗精神病药患者双侧眶额叶大脑半球间功能连接存在差异,且服药患者该功能连接与精神症状有相关性。Objective To investigate the characteristics of voxel-mirrored homotopic connectivity(VMHC)and its relationship with psychiatric symptoms in patient with drug-treated and drug-naive schizophrenia(SZ).Methods A total of 60 patients with SZ(divided into drug-treated patient group 40 cases and drug-naive patient group 20 cases)and 60 healthy controls(control group)were enrolled for resting state fMRI scanning and VMHC values were calculated.Results VMHC value in the orbitofrontal cortex,pallidum,middle occipital gyrus,precentral gyrus and postcentral gyrus showed significant differences among the three groups(P<0.05,Gaussian random field corrected).Pairwise comparisons in the post hoc analysis showed that compared with control group,VMHC value of the bilateral middle occipital gyrus,precentral gyrus and postcentral gyrus were decreased in drug-treated patient group(P<0.05),and VMHC values of the bilateral middle occipital gyrus,pallidum,precentral gyrus and postcentral gyrus was decreased in drugnaive patient group(P<0.05).Compared with drug-naive patient group,the drug-treated patient group showed decreased VMHC values in the bilateral orbitofrontal cortex(P<0.05).The VMHC value of bilateral orbitofrontal cortex was negatively correlated with the positive symptom score in drug-treated patient group(r=-0.45,P=0.01).Conclusions Interhemispheric functional connectivity(FC)in the sensorimotor network and visual network is reduced in patients with SZ.The bilateral orbitofrontal FC is different between drug-treated patient group and drug-naive patient group and is correlated with psychiatric symptoms in drug-treated patients.
