摘 要:目的探讨能级进阶在持续性肾替代治疗(CRRT)与体外膜肺氧合(ECMO)技能培训的实施效果。方法成立ECMO联合CRRT专业培训小组,选取46名ICU工作的护理人员,采用线上、线下混合授课的方式,一课2讲,并将课程录制成视频存储在钉钉专项学习资料包,供大家反复学习。理论考核采用问卷星、操作考核由专人考核,可保证评价效果的同质性。分析培训前后护理人员及实践技能掌握的情况及培训的满意度。结果不同层级护理人员培训前后知识的掌握情况得分比较差异有统计学意义(P<0.05);46名护理人员对ECMO联合CRRT的护理及专科操作技能水平熟练程度较佳;护理人员对培训及考核的满意度均较高。结论能级进阶模式应用在CRRT联合ECMO护理培训中能有效促进理论、技能的更新、拓展和提升,达到按能级培训的目的,提高培训效果。Objective To explore the implementation effect of clinical ladder in skills training of continuous renal replacement therapy(CRRT)combined with extracorporeal membrane oxygenation(ECMO).Methods A professional training group of CRRT combined with ECMO was formed.A total of 46 nurses working in ICU were trained online and offline,twice for one lesson,and the course was recorded into a video and stored in the DingTalk special learning package for everyone to learn repeatedly.Questionnaires were used for theoretical assessment,and operation assessment was conducted by dedicated persons,ensuring the homogeneity of evaluation effect.Mastery of practical skills before and after training and satisfaction with training were analyzed.Results There were statistical differences in mastery of knowledge scores among different levels of nurses before and after training(P<0.05).The 46 nurses were more proficient in nursing and professional operation skills of ECMO combined with CRRT,and they were highly satisfied with training and assessment.Conclusion The application of clinical ladder in CRRT combined with ECMO nursing training can effectively promote the renewal,expansion and improvement of theory and skills,achieve the purpose of training according to capability level and improve training effect.
