玻璃体切割联合内界膜剥除术治疗顽固性糖尿病黄斑水肿临床效果观察

摘　　要：目的:分析玻璃体切割联合内界膜剥除术治疗顽固性糖尿病黄斑水肿临床效果。方法:选择2016年12月16日~2019年12月16日某院所收治的105例顽固性糖尿病黄斑水肿患者为研究样本,依照患者是否存在后极部玻璃体后脱离情况,将其分为I组52例(65患眼、不存在玻璃体后脱离)和II组53例(67患眼、存在玻璃体后脱离),对患者开展玻璃体切割联合内界膜剥除术治疗,分析两组结果。结果:两组患者干预前后CMT相比,差异具有统计学意义(P<0.05);I组术后各个时间段的CMT和术前相比,差异具有统计学意义(P<0.05);II组患者术后1月和术前相比,CMT值存在统计学意义(P<0.05),手术后3月以及6月CMT和术前相比,差异无统计学意义(P>0.05);术后1、3、6月两组CMT相比,差异具有统计学意义(P<0.05)。干预前后两组BCVA差异具有统计学意义(P<0.05);I组术后各时间点BCVA和术前比较,差异具有统计学意义(P<0.05);II组术后各时间点BCVA和术前相比,差异无统计学意义(P>0.05);术后各时间点两组的BCVA对比,差异具有统计学意义(P<0.05)。两组患者术后未出现严重并发症。结论:针对无玻璃体后脱离的顽固性糖尿病黄斑水肿患者,应用PPV联合ILM剥除手术治疗疾病能够取得满意效果,有助于提升患者的视力水平。但如果病患玻璃体已发生脱离,且无显著牵拉情况时,开展此类手术治疗的效果并不显著。Objective:To analyze the clinical effect of vitrectomy combined with internal boundary membrane stripping in the treatment of refractory diabetic macular edema.Methods:A total of 105 patients with refractory diabetic macular edema admitted to a hospital from December 16,2016 to December 16,2019 were selected as the study samples.According to the presence or absence of posterior pole posterior vitreous detachment,the patients were divided into group I with 52 cases(65 eyes without posterior vitreous detachment)and group II with 53 cases(67 eyes with posterior vitreous detachment).The patients were treated with vitrectomy combined with internal boundary membrane stripping,and the results were analyzed.Results:There was statistically significant difference in CMT before and after intervention in the two groups(P<0.05);The CMT of group I at each time period after operation was different from that before operation,and the difference was statistically significant(P<0.05).The CMT value of group II was statistically significant at 1 month after surgery compared with that before surgery(P<0.05).The CMT values at 3 months and 6 months after operation were compared with that before operation,and the difference was not statistically significant(P>0.05).The CMT values at 1,3 and 6 months after operation were compared between the two groups,and the difference was statistically significant(P<0.05).There was statistically significant difference in BCVA before and after intervention between the two groups(P<0.05).The BCVA of group I at each time point after operation was compared with that before operation,and the difference was statistically significant(P<0.05).The BCVA of group II at each time point after operation was compared with that before operation,and the difference was not statistically significant(P>0.05).The BCVA at each time point after operation was compared between the two groups,and the difference was statistically significant(P<0.05).No serious complications occurred in the two groups after surgery.Conclusion:For
