显微根管治疗术治疗慢性根尖周炎的3年疗效评价

摘　　要：目的评估慢性根尖周炎显微根管治疗3年后的疗效,探讨临床工作中可能影响其疗效的因素:方法统计2017年1月至2017年12月诊断为慢性根尖周炎且由4名医师完成显微根管治疗术的患牙473例,于术后3年回访,通过影像学检查及临床指标评估患牙治疗效果,同时对其影响因素进行相关性分析。结果总体治疗成功率为82.9%,再治疗成功率为75.8%,初次根管治疗成功率为86.1%,差异有统计学意义(P<0.001);显微根管治疗失败与根管治疗质量、牙位、治疗后是否冠部修复以及操作医师具有相关性(P>O.OOl)。年龄,性别,根管预备器械,根尖炎症范围大小与治疗成功与否无明显相关性(P>0.05)。结论临床工作中应规范显微镜及显微器械的使用,以提高感染控制。Objective To evaluate the effect of micro root canal treatment on chronic apical periodontitis after 3 years.Methods A total of 473 teeth diagnosed as chronic periapical periodontitis were treated with micro root canal therapy by 4 dentists.After 3 years'follow-up,the treatment effect was evaluated by imaging examination and clinical indicators.Results The overall success rate was 82.9%,the success rate of retreatment was 75.8%,and the success rate of initial treatment was 86.1%.The difference was significant(P<0.001).There was no significant correlation between age,gender,root canal preparation instruments,the extent of apical inflammation and the success of treatment(P>0.05).Conclusions We should standardize the use of microscope,give full play to its advantages to improve infection control.
