口服2.5%等渗甘露醇溶液联合MSCT行肠道CT造影在大肠癌术前TNM分期评估中的应用价值

摘　　要：目的研究口服2.5%甘露醇等渗液联合多排螺旋CT(MSCT)行肠道CT造影在大肠癌术前TNM分期评估中的应用价值。方法选择2019年1月至2020年12月延安大学咸阳医院收治的82例大肠癌患者为研究对象,按照随机数表法分为对照组42例和研究组40例,对照组患者仅采用64-MSCT检查,研究组患者联合口服2.5%等渗甘露醇行肠道CT造影检查,根据检查结果对所有患者进行术前TNM分期,将手术病理结果与检查结果进行对比,计算出两种方案的诊断效能。结果研究组患者的T≤2期特异度为100.00%,明显高于对照组的51.72%,差异有统计学意义(P<0.05);研究组患者的T4期特异度为97.22%,明显高于对照组的63.16%,差异有统计学意义(P<0.05);研究组患者T≤2期和T4期诊断准确率分别为100.00%、92.50%,明显高于对照组的59.52%、61.90%,差异均有统计学意义(P<0.05);研究组患者转移分期N0和N1~2诊断准确率分别为87.50%、92.50%,明显高于对照组的80.95%、69.06%,差异均有统计学意义(P<0.05)。结论MSCT肠道造影检查前口服2.5%等渗甘露醇溶液可有效提高临床对大肠癌术前分期评估的准确率,对疾病分期的评估具有重要意义。Objective To study the application of 2.5%mannitol isotonic solution combined with multi-slice spiral CT(MSCT)in the preoperative TNM staging evaluation of colorectal cancer.Methods Eighty-two patients with colorectal cancer who were treated in Xianyang Hospital of Yan'an University from January 2019 to December 2020 were selected as the research objects.The patients in the control group were divided into a control group(42 patients)and a study group(40 patients)according to the random number table method.Patients in the control group only received 64-MSCT examination,while patients in the study group received 64-MSCT examination combined with oral 2.5%isoosmotic mannitol for CT examination.According to the results of the examination,all patients were divided into groups with different TNM stages.The results of operation pathology were compared with the results of examination,and the diagnostic efficacy of the two schemes was calculated and compared with the histopathological results.Results The specificity of T≤2 in the study group was 100.00%,which was significantly higher than 51.72%in the control group(P<0.05).The specificity of T4 in the study group was 97.22%,which was significantly higher than 63.16%in the control group(P<0.05).The diagnostic accuracy of T≤2 and T4 in the study group were 100.00%and 92.50%,respectively,which were significantly higher than 59.52%and 61.90%in the control group(P<0.05).The accuracy rates of N0 and N1-2 in the study group were 87.50%and 92.50%,respectively,which were significantly higher than 80.95%and 69.06%in the control group(P<0.05).Conclusion Before MSCT enterography,oral administration of 2.5%isotonic mannitol solution can effectively improve the accuracy of preoperative staging evaluation of colorectal cancer,which is of great significance to the evaluation of disease staging.
