越鞠保和丸联合温针灸治疗抑郁症临床观察

摘　　要：目的:观察越鞠保和丸联合温针灸治疗抑郁症的临床疗效及对5-羟色胺(5-HT)、多巴胺(DA)、脑电图α波的影响。方法:将78例抑郁症患者随机分为两组各39例,治疗组予越鞠保和丸口服联合温针灸治疗,对照组予阿米替林口服治疗;两组疗程均为8周。观察并比较两组临床疗效、各量表评分、血清中5-HT、DA水平及脑电图α波的变化。6个月后进行远期疗效评定。结果:治疗组近期总有效率为91.67%,远期总有效率为87.50%;分别优于对照组的71.43%和63.33%(均P﹤0.05)。两组治疗前后HAMD、HAMA、TESS评分和脑电图α波频率、波幅组内比较,差异有统计学意义(P﹤0.05),治疗后组间比较,差异有统计学意义(P﹤0.05)。治疗后,两组血清5-HT、DA水平均显著升高,且治疗组水平显著高于对照组(均P﹤0.05)。结论:越鞠保和丸联合温针灸治疗抑郁症,能明显改善患者抑郁、焦虑程度,提高血清5-HT、DA的表达水平,影响脑电图α波的变化,临床疗效较好。Objective:To observe the clinical efficacy of Yueju Baohe pill and warming needle moxibustion on depression and its effect on 5-hydroxytryptamine(5-HT),dopamine(DA)and EEGαwave.Methods:78 cases of patients with depression were randomly divided into two groups,39 cases in each group.The treatment group was treated with oral Yueju Baohe pill and warming needle moxibustion,while the control group was treated with oral amitriptyline.After 8 weeks of treatment,the clinical efficacy and the score of each scale were observed in two groups,the levels of 5-HT and DA in serum were detected,and the change of EEGαwave was recorded.After 6 months,the long-term efficacy was evaluated.Results:After treatment in treatment group,the short-term effective rate was 91.67%and the long-term effective rate was 87.50%,which was better than 71.43%and 63.33%in control group respectively(all P﹤0.05).The scores of HAMD,HAMA and TESS as well as the frequency and amplitude of EEGαwave were compared before and after treatment respectively,and the differences in two groups were statistically significant(P﹤0.05);the differences between two groups after treatment were also statistically significant(P﹤0.05).After treatment,the levels of 5-HT and DA in serum were significantly increased in both groups,and the levels of treatment group were significantly higher than those of control group(all P﹤0.05).Conclusion:Yueju Baohe pill and warming needle moxibustion for treatment of depression can significantly improve the degrees of depression and anxiety of patients,increase the expression levels of 5-HT and DA in serum,affect the change of EEGαwave,and with better clinical efficacy.
