外伤性眼内炎临床特征及影响术后视力因素的分析

摘　　要：目的分析外伤性感染性眼内炎临床特征及影响术后视力的因素。方法回顾性分析山西省眼科医院2014年1月至2018年12月收治的外伤性眼内炎246例(246只眼)的病例资料。观察外伤性眼内炎的临床特征,并对影响术后视力的有关因素进行多因素logistic回归分析。结果患者年龄为5~84(41.67±15.51)岁,男女比值为8.1∶1,职业以工人占比最多。眼外伤类型以眼球穿孔伤多见,眼内异物以金属性者为主,外伤分区以Ⅰ区最多见,Ⅲ区的基线视力最差。细菌培养阳性率为40.7%(90/227)。患者的中位基线视力(logMAR)为2.3(2.3,2.7),治疗后明显提高至1.8(0.9,2.3)(Z=-10.398,P<0.001)。眼内异物是影响术后视力的因素。结论眼内异物的存在是影响外伤性眼内炎术后视力的独立因素。不同的治疗方式与就诊时间有相关性。外伤性眼内炎致病菌阳性率为40.7%,最常见致病菌为表皮葡萄球菌。Objective To analyze the clinical characteristics of traumatic endophthalmitis and the related influence factors on postoperative visual acuity.Methods The data of 246 eyes of 246 patients with traumatic endophthalmitis from Jan.2014 to Dec.2018 in Shanxi Eye Hospital were analyzed retrospectively.The epidemiological characteristics of traumatic endophthalmitis and the influence factors related to visual acuity were analyzed.Results The ages of patients with traumatic endophthalmitis were ranged from 5 to 84 years,with average of(41.67±15.51)years.The male/female ratio was 8.1∶1,and labour workers accounted for the largest proportion of occupations.The type of trauma was mostly common in eyeball lacerations.Foreign bodies in the eye were mainly metallic foreign bodies,and the mostly common trauma area was zone 1.And the baseline visual acuity of areaⅢwas the worst.The positive rate of bacterial culture was 40.7%(90/221).The mean baseline visual acuity of patients(logMAR)was 2.3(2.3,2.7),which improved to 1.8(0.9,2.3)(Z=-10.398,P<0.001)postoperatively.The presence of foreign bodies in the eye was a risk factor for impaired vision.Conclusion The presence of foreign bodies in the eye is an independent risk factor for visual acuity in traumatic endophthalmitis.There is a correlation between treatment methods and visiting time.The positive rate of pathogenic bacteria in traumatic endophthalmitis is 40.7%,and the most common causative organisms is Staphylococcus epidermidis.
