摘 要:目的探讨戴角膜塑形镜者近距离用眼时长对眼轴长度的影响。方法回顾性临床研究。纳入2018年1月至2020年1月在郑州大学第一附属医院视光中心佩戴角膜塑形镜的少年儿童90例(180眼),依据每天不同近距离用眼时长分为A、B、C 3组,每组各30例(60眼)。每天近距离用眼时间6 h以下者归为A组、6~9 h者归为B组、9 h以上者归为C组。3组间的男女比例及年龄大小差异无统计学意义。比较3组之间1年的眼轴长度增加值有无差异。结果眼轴增加量:A、B两组间,B、C两组间差异均无统计学意义(q=0.156、-1.572,P=0.877、0.127),A组少于C组(q=-2.105,P=0.044)。眼轴增加量与近距离用眼时长相关性分析:A组两者间无明显相关性(r=0.082,P=0.730),B组两者间呈弱正相关性(r=0.409,P=0.038)、C组两者间呈正相关(r=0.667,P=0.003)。结论佩戴角膜塑形镜的少年儿童近视者,其近距离用眼超过一定时长者才会导致眼轴的明显增加。Objective To investigate the effect on axial length of different time of short distances eye-use of the myopics wearing orthokeratolologic lens.Methods This was a retrospective clinical study.Total of 180 eyes of 90 cases who wear orthokeratolologic lens from Jan.2018 to Jan.2020 in the optometry center of the First Affiliated Hospital of Zhengzhou University were included in this study.They were divided into three groups based on the time of short distance eye-use,with 60 eyes of 30 cases in each group.Group A used eyes in short distance for less than 6 hours per day,Group B used eyes for 6-9 hours and Group C used eyes for more than 9 hours.The difference of male and famale and their age among the three groups were not statistically significant.The increase of axial length after one year among the three groups were compared.Results Analysis of axial increment:there was no significant difference between group A and group B,and between group B and group C(q=0.156,-1.572;P=0.877,0.127),but that of group A was less than that of group C(q=-2.105,P=0.044).Analysis of the relationship between the increase of axial length and the time of short distance eye-use:there was no significant correlation between them in group A(r=0.082,P=0.730),there was a weak positive correlation between them in group B(r=0.409,P=0.038)and a positive correlation in group C(r=0.667,P=0.003).Conclusion The increase in axial length is not apparent until the time of short distance eye-use is over a certain threshold in adolescents with myopia who wear orthokeratologic lens.
