摘　　要：目的探讨泪道探通引导针联合Ritleng泪道支撑管置管术治疗鼻泪管阻塞的效果。方法回顾性系列病例研究。收集同济大学附属同济医院2018年6月至2020年8月鼻泪道阻塞25例(32眼)接受泪道探通引导针联合Ritleng泪道支撑管置管术,术后随访6个月,对术中操作、手术效果及并发症进行分析。结果术后3个月拔管,拔管后3个月泪道冲洗通畅率A级(通畅)20眼(62.50%),B级(通,稍有不畅)4眼(12.50%),C级(通而不畅)3眼(9.38%),D级(不通)5眼(15.62%)。术前、术后1周、3个月及6个月,患者泪河高度依次为(528.63±65.23)μm、(296.72±248.53)μm、(256.36±187.62)μm及(271.81±192.43)μm。结论泪道探通引导针联合Ritleng泪道支撑管治疗鼻泪管阻塞具有操作简单、快速,并发症少等优点。Objective To investigate the clinical efficacy of lacrimal duct probing combined with Ritleng stent intubation in the treatment of nasolacrimal duct obstruction.Methods This was a retrospective study.Total of 32 eyes of 25 patients with nasolacrimal duct obstruction from Jun.2018 to Aug.2020 in Tongji Hospital Affiliated to Tongji University were treated with lacrimal probing combined with Ritleng stent intubation.The follow-up period was 6 months.The intraoperative manipulation,postoperative efficacy and complications were analyzed.Results All the surgery were performed successfully.At 3 months after extubation,the lacrimal irrigating patency rate was as grade A(unobstructed without resistance)in 20 eyes(62.50%),grade B(unobstructed with resistance)in 4 eyes(12.50%),grade C(partial reflux,pharyngeal with water through)in 3 eyes(9.38%)and grade D(complete reflux,pharyngeal without water through)in 5 eyes(15.62%).Before operation,1 week,3 months and 6 months after operation,the tear meniscus height were(528.63±65.23)μm,(296.72±248.53)μm,(256.36±187.62)μm and(271.81±192.43)μm,respectively.Conclusion Lacrimal duct probing combined with Ritleng stent intubation in the treatment of nasolacrimal duct obstruction disease has the advantages of simple operation and fewer postoperative complications.
