摘  要:目的探讨131Ⅰ治疗对甲状腺乳头状癌(PTC)患者外周血淋巴细胞染色体畸变率(CAR)和甲状腺球蛋白抗体(Tg Ab)变化的影响。方法选取2020年1月至2020年12月间宝鸡高新医院接受治疗的62例PTC患者作为研究对象,并根据131Ⅰ的剂量分为高剂量组(26例)和低剂量组(36例),分析131Ⅰ治疗效果以及不同剂量治疗前1d、治疗后5d、治疗后1个月和治疗6个月后外周血淋巴细胞CAR和Tg Ab的变化情况。结果62例患者经131Ⅰ治疗后,完全治愈4例(6.5%),好转30例(48.4%),无效28例(45.2%),临床总有效率为54.8%,临床无效率为45.2%。治疗后5d的CAR和Tg水平高于治疗后1个月,治疗后1个月后的CAR和Tg水平高于治疗前1d和治疗后6个月;治疗后5d的微核率(MNR)水平高于治疗后1个月,治疗后1个月的MNR水平高于治疗后6个月,治疗后6个月的MNR水平高于治疗前1d;治疗后6个月的Tg Ab水平低于治疗后1个月,治疗后1个月的Tg Ab水平低于治疗后5d,治疗后5d的Tg Ab水平低于治疗前1d,差异均有统计学意义(均P<0.05)。高剂量组治疗后5d、治疗后1个月、治疗后6个月CAR水平高于低剂量组,高剂量组治疗后5d、治疗后1个月和治疗后6个月的Tg Ab水平低于低剂量组,差异均有统计学意义(均P<0.05)。结论131Ⅰ治疗对PTC患者可导致外周血淋巴细胞CAR增大,且剂量越大,CAR增大越明显,但这种增大是暂时性,可随着时间的延长逐渐恢复;Tg Ab在131Ⅰ治疗后显著降低,且131Ⅰ剂量越高降低越明显。Objective To explore the effect of^(131)Ⅰ on changes in chromosome aberration rate(CAR)and thyroglobulin antibodies(Tg Ab)in peripheral blood lymphocytes in patients with papillary thy-roid carcinoma(PTC).Methods Sixty-two PTC patients admitted to Baoji High-tech Hospital were select-ed from January 2020 to December 2020.They were divided into a high dose group(26 patients)and a low dose group(36 patients)according to the dosage of131Ⅰ.The therapeutic effect of131Ⅰ and the changes in CAR and Tg Ab in peripheral blood lymphocytes with different doses of^(131)Ⅰ were analyzed at 1 d before the treatment,5 d after the treatment,1 month after the treatment and 6 months after the treatment.Results After^(131)Ⅰ treatment,4 patients(6.5%)were completely cured,condition was improved in 30 patients(48.4%),and the treatment was ineffective in 28 patients(45.2%).The overall clinical efficacy was 54.8%.The levels of CAR and Tg were significantly higher at 5 d after the treatment than at 1 month after the treatment,and higher at 1 month after the treatment than at 1 d before the treatment and 6 months after the treatment(all P<0.05).The levels of micronucleus rate(MNR)were significantly higher at 5 d after the treatment than at 1 month after the treatment,higher at 1 month after the treatment than at 6 months after the treatment and higher at 6 months after the treatment than at 1 d before the treatment.The levels of Tg Ab were significantly lower at 6 months after the treatment than at 1 month after the treatment,lower at 1 month after the treatment than at 5 d after the treatment and lower at 5 d after the treatment than at 1 d after the treatment(all P<0.05).The CAR levels were significantly higher in the high dose group than in the low dose group at 5 d,1 month and 6 months after the treatment(all P<0.05).Tg Ab levels were lower in the high dose group than in the low dose group at 5 d,1 month and 6 months after the treatment(all P<0.05).TGAb levels in high-dose group were significantly lower than those in low-dos
