摘  要:目的:通过micro-CT扫描检查,对下颌第二磨牙根管走行中出现汇合与分叉的概率、位置及成角的规律进行分析。方法:选取本院口腔科新鲜拔除的46颗下颌第二磨牙为研究对象,离体牙均采用micro-CT进行扫描,并行三维重建。结合三维可视化图像,定性和定量分析了根管的形态、分支、汇合和分叉的发生率、位置及成角情况。结果:根管形态学分析显示,离体牙样本中有19颗为C形根管,占比41.30%;27颗为非融合根管,占比58.70%。共有31颗牙存在分支(72个分支)根管,其中56个分支(77.78%)位于根尖1/3,16个分支(22.22%)位于根中1/3。C形根管中发生汇合的17个,发生分叉的15个,汇合和分叉并存的14个。C形根管分叉时的成角为(34.8±8.45)°,明显高于非融合根管(26.2±3.14)°,差异具有统计学意义(P=0.001)。结论:下颌第二磨牙根管系统非常复杂,利用micro-CT扫描研究根管的分支、汇合和分叉以及成角规律,对于提高根管治疗的成功率意义重大。Objective:Through the micro-CT scan,the probability,location and angulation of the root canals of the mandibular second molars were analyzed.Methods:Forty-six mandibular second molars freshly extracted from patients in the Department of Stomatology of our hospital were selected as the research object.The isolated teeth were scanned by micro-CT and three-dimensional reconstruction was performed in parallel.Combined with three-dimensional visualization images,qualitatively and quantitatively analyzed the root canal morphology,branching,confluence and bifurcation incidence,location and angle.Results:Root canal morphology analysis showed that 19 teeth in the isolated tooth samples were C-shaped root canals,accounting for 41.30%,27 teeth were non-fusion root canals,accounting for 58.70%.A total of 31 teeth have branches(72 branches)root canals,of which 56 branches(77.78%)are located at 1/3 of the root tip and 16 branches(22.22%)are located at 1/3 of the root.Among the C-shaped root canals,17 converged,15 bifurcated and 14 converged and bifurcated.The angle of C-shaped root canal bifurcation was(34.8±8.45)°,which was significantly higher than that of non-fusion root canal(26.2±3.14)°,and the difference was statistically significant(P=0.001).Conclusion:The root canal system of the mandibular second molars is very complicated.Using micro-CT scanning to study the branching,confluence and bifurcation of root canals,is of great significance for improving the success rate of root canal treatment.
