摘　　要：目的:探讨甘油三酯-葡萄糖(TyG)指数对急性胰腺炎(AP)严重程度的早期预测价值。方法:选取2017年6月~2020年6月我院收治的AP患者作为研究对象,收集临床资料,计算TyG指数,将患者按严重程度分为轻度AP、中重度AP和重度AP(SAP)。非SAP组患者164例。SAP组21例。比较2组的临床基本特征,采用多因素Logistic回归分析SAP发生的独立危险因素,ROC曲线比较TyG指数和PCT对SAP的预测价值。结果:SAP组的TG、TyG指数和PCT水平显著高于非SAP组(均P<0.01)。SAP组的住院天数、入住ICU的比例和死亡率显著高于非SAP组(均P<0.01)。高水平的TyG指数和高水平的PCT是AP患者发生SAP的独立危险因素。TyG指数预测SAP的AUC为0.858(95%CI 0.743~0.974,P<0.01),显著高于PCT的AUC[0.769(95%CI0.632~0.907,P<0.01)]。结论:TyG指数具有更高的检验效能。高水平的TyG指数是AP患者发生SAP的独立危险因素。Objective:To investigate the early predictive value of triglyceride and glucose index(TyG index) in the severity of acute pancreatitis(AP).Methods:Patients with AP admitted to our hospital from June 2017 to June 2020 were selected as the research subjects.The clinical data of these AP patients were collected,and the admission TyG index was calculated.They were classified into mild,moderate and severe patients according to the severity,and grouped into SAP(severe AP)group and non-SAP(mild and moderate AP)group.There were164 cases in non-SAP group and 21 cases in SAP group.The basic clinical characteristics of the patients in SAP group and non-SAP group were compared.The independent risk factors of SAP were analyzed by multivariate logistic regression.The predictive value of TyG index and procalcitonin(PCT)for SAP were compared by ROC curve.Results:The triglyceride(TG),TyG index and PCT levels in SAP group were significantly higher than those in non-SAP group.In addition,the number of hospital stays,the percentage of ICU admissions and mortality in the SAP group were significantly higher than those in the non-SAP group.High level of TyG index and high level of PCT were independent risk factors for SAP patients.The AUC of TyG index predicted that SAP was 0.858(95%CI 0.743-0.974,P<0.01),significantly higher than that of PCT [0.769(95%CI 0.632-0.907,P<0.01)].Conclusion:TyG index has higher inspection efficiency.High level of TyG index is an independent risk factor for SAP in patients with AP.
