摘 要:目的:观察放射性^(125)I粒子植入联合化疗治疗非小细胞肺癌的近期临床疗效,探讨临床应用价值。方法:选取非小细胞肺癌患者82例,按照随机数字表法随机分为观察组41例和对照组41例。观察组采用^(125)I粒子植入术联合化疗,化疗方案为DDP联合盐酸吉西他滨。对照组采用单纯化疗,化疗方案与观察组相同。观察两组临床疗效及不良反应。结果:观察组总有效率为63.41%、6个月临床控制率为85.37%、KPS总有效率为75.61%,均显著高于对照组的43.90%、63.41%、51.22%,差异具有统计学意义(P<0.05)。观察组1年和2年生存率明显高于对照组,差异具有统计学意义(P<0.05)。观察组所有并发症可控,无患者粒子脱靶易位和放射性食管损伤。两组在恶心呕吐、发热、谷丙转氨酶升高、骨髓抑制发生率方面比较,差异无统计学意义(P>0.05)。结论:^(125)I粒子植入联合化疗有较好的近期疗效,且未增加不良反应,是一种有效的治疗NSCLC方案。Objective To investigate the short-term efficacy and explore the clinical value of^(125)I radioactive seeds implantation for treatment of advanced non-small cell lung cancer.Method Using the digital table method,82 patients diagnosed with middle late stage non-small cell lung cancer by clinic,were randomized into observation group(n=41)and control group(n=41).The control group was treated with GP scheme chemotherapy,compared that the observation group was treated with^(125)I seeds implantation.The clinical,survival rates and incidence of adverse reaction were analyzed.Results The effective rate,six month local control rate and KPS total efficiency in observation group 63.41%,85.37%and 75.61%was significantly higher than that in control group 43.90%,63.41%and 51.22%(P<0.05).For the survival rate,there were significant difference 1 and 2 year survival rate between two groups(P<0.05),and the survival rate of control group is lower.All the side effects were manageable,neither radiographic esophageal injury nor iodine miss translocation.There was no difference between them in nausea and vomiting,fever,Cereal third transaminase lifts and bone marrow depression(P>0.05).Conclusion The^(125)I seed implantation Brach therapy combined with chemotherapy shows an acceptable toxicity,and good therapeutic effectiveness,and is an effective and satisfactory therapeutic modality in the treatment of advanced non-small cell lung cancer.
