e-PTFE额肌悬吊术治疗儿童先天性上睑下垂的效果分析

摘　　要：目的探讨e-PTFE额肌悬吊术在儿童先天性上睑下垂中的效果。方法选取先天性上睑下垂患儿72例的临床资料进,依据患儿手术方案分为试验组(n=36,e-PTFE额肌悬吊术)与对照组(n=36,额肌瓣悬吊术)。比较手术前后矫治效果和手术并发症,观察手术前后睑裂高度、美学效果评分。结果试验组术后矫正率为94.44%,较对照组的88.89%升高(P<0.05)。术后2组睑裂高度先升高后降低,差异显著(P<0.05),但是两组间比较,差异无统计学意义(P>0.05).术后3个月2组患者美学效果评分(双眼睑宽度对称、双眼睑高度对称、上睑轮廓)较术前显著升高(P<0.05),术后3个月试验组高于对照组(P<0.05)。两组并发症比较,无明显差异性(P>0.05)。结论先天性上睑下垂患儿的临床治疗中采用e-PTFE额肌悬吊术治疗效果显著,可有效改善患儿眼睑状况,且改善患儿外观,同时手术并发症相对较少,临床可进行推广应用。Objective To investigate the effect of e-PTFE frontal muscle suspension in children with congenital ptosis.Methods The clinical data of 72 children with congenital blepharoptosis were selected for a retrospective study.The children were divided into experimental group(n=36,e-PTFE frontal muscle suspension)and control group(n=36,frontal muscle suspension)according to the surgical plan.Muscle flap suspension.The correction effect before and after the operation and the complications of the operation were compared,and the height of the palpebral fissure before and after the operation and the score of the aesthetic effect were observed.Results The postoperative correction rate in the experimental group was 94.44%,which was higher than the 88.89%in the control group(P<0.05).The height of the palpebral fissure first increased and then decreased in the two groups after surgery,the difference was significant(P<0.05),but the difference between the two groups was not statistically significant(P>0.05).The aesthetic effect score of the two groups was 3 months after surgery.The double eyelid width was symmetrical,the double eyelid height was symmetrical,and the upper eyelid contour was significantly higher than that before operation(P<0.05),and the test group was higher than the control group at 3 months after operation(P<0.05).There was no significant difference in complications between the two groups(P>0.05).Conclusion In the clinical treatment of children with congenital ptosis,the e-PTFE frontal muscle suspension has a significant therapeutic effect.It can effectively improve the condition of the eyelid and the appearance of the child.At the same time,there are relatively few surgical complications,and it is clinically feasible.Promote applications.
