摘　　要：目的探讨激光周边虹膜切除术(LPI)对眼外伤继发青光眼单核细胞趋化蛋白-1(MCP-1)的影响。方法118例(118眼)眼外伤继发青光眼患者,根据接受的手术类型分组,观察组(n=58)行LPI,对照组(n=60)行小梁切除术。比较两组疗效及术前、术后3 d时玻璃体MCP-1、高迁移率族蛋白B1(HMGB1)、血清炎症因子[白介素-2(IL-2)、可溶性白介素-2受体(s IL-2R)、干扰素-γ(IFN-γ)]水平,术后并发症发生情况。结果两组总有效率比较,差异无统计学意义(P>0.05);治疗3d后,两组玻璃体MCP-1、HMGB1水平、血清IL-2、s IL-2R、IFN-γ水平均较术前上升,观察组指标低于对照组(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论LPI、小梁切除术治疗眼外伤继发青光眼疗效均较好,但LPI对患者玻璃体MCP-1、HMGB1和血清炎症因子水平影响更小,且两种治疗方式的患者术后并发症差异不显著。Objective To explore the effect of laser peripheral iridectomy(LPI)onmonocyte chemoattractant protein-1(MCP-1)in the treatment of glaucoma secondary to ocular trauma.Methods One hundred and eighteen patients(118 eyes)with glaucoma secondary to ocular traumawere divided into an observation group and a control group according to the type of surgery.The observation group(n=58)received LPI and the control group(n=60)performed trabeculectomy.The efficacy,the levels ofvitreous MCP-1,high mobility group box 1(HMGB1),and serum inflammatory factors such as interleukin-2(IL-2),soluble interleukin-2 receptor(s IL-2 R)and interferon-γ(IFN-γ)before surgery and after 3 daysof surgery as well as occurrence of postoperative complications were compared between the two groups of patients.Results There was no significant difference in the total effective rate between the two groups(P>0.05).After 3 days of treatment,the levels of vitreous MCP-1,HMGB1,serum IL-2,s IL-2 R and IFN-γin the two groups were increased compared with those before surgery,but the levels of the observation group were lower than those of the control group(P<0.05).There was no significant difference in the incidence rate of postoperative complications between the two groups(P>0.05).Conclusion Both LPI and trabeculectomy have a better effect in the treatment of glaucoma secondary to ocular trauma,but LPI has less effect on the levels of vitreous MCP-1,HMGB1 and serum inflammatory factors.There are no significant differences in postoperative complications between the two treatment methods.
