摘　　要：目的:探究自酸蚀粘接修复系统对老年重度磨耗患者牙列外观及修复效果的影响。方法:选取2017年1月-2019年3月笔者医院就诊的104例老年重度磨耗患者的临床资料,开展回顾性研究,根据治疗方案不同将患者分为两组,每组52例。研究组采用自酸蚀粘接修复治疗,对照组采用全酸蚀粘接修复治疗。比较两组治疗成功率、治疗前后牙齿敏感性、牙列外观、咀嚼功能、口腔卫生指数[简化软垢指数(DI-S)、简化牙石指数(CI-S)]及生活质量(GOHAI)评分。结果:研究组治疗成功率94.23%,略高于对照组的88.46%,但差异无统计学意义(P>0.05);治疗1个月、6个月后,研究组牙齿敏感性优于对照组(P<0.05);治疗1个月、6个月、12个月后,研究组牙列外观、咀嚼功能评分高于对照组(P<0.05);治疗6个月、12个月后,研究组DI-S、CI-S评分低于对照组(P<0.05);治疗1个月、6个月、12个月后,研究组生理功能、疼痛与不适、社会心理功能评分及GOHAI总分高于对照组(P<0.05)。结论:采用自酸蚀粘接修复治疗老年重度磨耗患者,治疗成功率高,能有效降低牙齿敏感性,显著改善患者牙列外观及修复效果,从而提高患者生活质量。Objective To investigate the effect of self-etching adhesive repair system on the appearance and repair effect of dentition in elderly patients with severe abrasion.Methods The clinical data of 104 elderly patients with severe abrasion in our hospital from January 2017 to March 2019 were selected to conduct a retrospective study.The patients were divided into two groups according to different treatment plans,with 52 cases in each group.The study group was treated with self-etching adhesive repair,and the control group was treated with full-etching adhesive repair.Comparing the treatment success rate,tooth sensitivity,dentition appearance,chewing function,and oral hygiene index[simplified soft scale index(DI-S),simplified tartar index(CI-S)],quality of life(GOHAI score)before and after treatment between the two groups.Results The treatment success rate of the study group(94.23%)was slightly higher than that of the control group(88.46%),but the difference was not statistically significant(P>0.05),the tooth sensitivity of the study group was better than that of the control group at 1 and 6 months of treatment(P<0.05),the scores of dentition appearance and chewing function after treatment for 1 month,6 months,and 12 months in the study group were higher than those in the control group(P<0.05).The DI-S and CI-S scores of the study group were lower than those of the control group after 6 months and 12 months of treatment(P<0.05).The scores of physiological function,pain and discomfort,psychosocial function and GOHAI scores of the study group were higher than those of the control group after 1 month,6 months,and 12 months of treatment(P<0.05).Conclusion The self-etching adhesive restoration is used to treat elderly patients with severe abrasion.The treatment success rate is high.It can effectively reduce tooth sensitivity,significantly improve the appearance of the patient's dentition and the effect of restoration,thereby improving the quality of life of the patient.
