摘　　要：目的观察视神经脊髓炎谱系疾病(NMOSD)患者血清和脑脊液尿酸(UA)水平变化,并探讨其临床意义。方法纳入30例NMOSD患者(NMOSD组)、23例非炎症性神经系统疾病患者(NIND组)、26例健康对照者(HC组),采用尿酸酶法检测治疗前血清和脑脊液UA水平。比较各组血清、脑脊液UA水平及不同临床特征的NMOSD患者血清、脑脊液UA水平;采用Spearman相关性分析脑脊液UA与血清UA水平的相关性。结果NMOSD组血清UA水平低于NIND组和HC组(P均<0.05);NMOSD组脑脊液UA水平高于NIND组,但差异无统计学意义(P=0.153)。不同临床特征的NMOSD患者血清、脑脊液UA水平比较,P均>0.05。在NMOSD患者中,脑脊液UA水平与血清UA水平呈正相关(r=0.495,P=0.005)。结论NMOSD患者血清UA水平降低,脑脊液UA水平未见降低。血清、脑脊液UA水平与NMOSD患者临床病理特征无关。脑脊液UA与血清UA水平有正相关关系。Objective To observe the changes in levels of uric acid(UA)from serum and cerebrospinal fluid in pa-tients with neuromyelitis optica spectrum disorders(NMOSD)and to investigate the clinical significance.Methods Thirty NMOSD patients(NMOSD group),23 non-inflammatory neurological disorders patients(NIND group),and 26 healthy controls(HC group)were included in this study.Uricase method was performed to detect cerebrospinal fluid and serum UA levels before the treatment.We compared the differences in the cerebrospinal fluid and serum UA levels be-tween these three groups,as well as between NMOSD patients with different clinical characteristics.Spearman correlation was used to analyze the correlation between cerebrospinal fluid UA and serum UA levels.Results The serum UA level in the NMOSD group was significantly lower than those in the NIND and HC groups(both P<0.05).The cerebrospinal flu-id UA level in the NMOSD group was higher than that in the NIND group,without a statistical significance(P=0.153).No statistically significant differences were found in the comparisons of serum and cerebrospinal fluid UA levels between NMOSD patients with different clinical characteristics(all P>0.05).There was a positive correlation between serum and cerebrospinal fluid UA levels in NMOSD patients(r=0.495,P=0.005).Conclusions In NMOSD patients,the cerebro-spinal fluid UA level did not decrease while the serum UA level is reduced.There are no correlations between clinical char-acteristics and the UA levels from serum and cerebrospinal fluid in NMOSD patients.There is a positive correlation be-tween cerebrospinal fluid and serum UA levels in NMOSD patients.
