妊娠期糖尿病产妇泌乳Ⅱ期启动延迟发生率及影响因素的系统评价

摘　　要：目的系统评价妊娠期糖尿病产妇泌乳Ⅱ期启动延迟的发生率及影响因素,为相关研究和干预提供借鉴。方法计算机检索Web of Science、PubMed、EMBASE、Cochrane、维普、中国知网、万方和中国生物医学文献数据库中妊娠期糖尿病产妇泌乳Ⅱ期启动延迟发生率及影响因素的相关研究文献;两名研究者对文献进行质量评价和资料提取,进行系统评价;符合量性分析的资料使用Stata 12.0行Meta分析。结果基于9项研究Meta分析显示,泌乳Ⅱ期启动延迟发生率为34%[95%CI(0.29,0.39)],主要影响因素可能包括产妇因素:年龄、体质指数、产次、分娩方式、焦虑、抑郁、胰岛素使用;婴儿因素:早产、胎龄;社会因素:医院母乳喂养支持等。结论妊娠期糖尿病产妇泌乳Ⅱ期启动延迟发生率较高,其影响因素相关研究较少,且部分因素的作用仍有不同评价,与泌乳Ⅱ期启动延迟的因果关系有待进一步验证。To systematically evaluate the incidence and influencing factors of DOL II among pregnant women with gestational diabetes mellitus,and to provide a scientific basis for research and formulating intervention measures.Methods Web of Science,PubMed,EMBASE,Cochrane,CBM,WF,VIP and CNKI databases were searched to collect the references on the incidence and influencing factors of DOL II,the quality of references was critically appraised and the data were extracted by two investigators independently.Meta-analysis was conducted using Stata12.0 software.Results 9 references were included.The incidence of DOL II was 34%[95%CI(0.29,0.39)].The influencing factors may include maternal factors:age,body mass index,parity,delivery method,anxiety,depression,pregnancy comorbidities,insulin use,infant factors:premature birth,gestational age;social factors:hospital breastfeeding support.Conclusion The incidence of DOLⅡamong pregnant women with gestational diabetes mellitus is relatively high.There are few related studies on its influencing factors,and there are still different opinions on some factors.The causal relationship between the factors and DOL II needs to be further verified.
