摘　　要：目的:探讨卵巢卵泡膜细胞瘤的磁共振成像(magnetic resonance image,MRI)表现。方法:回顾性分析经临床手术病理证实卵泡膜细胞瘤26例的MRI资料,分析病灶的部位、形态、边界、信号、表观扩散系数(apparent diffusion coefficient,ADC)值测量、强化方式、有无合并盆腔积液和子宫内膜改变。结果:26例病灶均为单发,边界清,类圆形肿块。8例呈实性,MRI平扫T1WI呈等或稍低信号,T2WI呈等或稍高信号,T2WI抑脂序列呈高信号;18例呈囊实性改变,T2WI呈混杂高信号。所有病例扩散加权成像均呈稍高信号,平均ADC值为1.380×10^(-3) mm^(2)/s;增强扫描呈渐进性强化,但仍低于子宫肌层信号。合并子宫内膜增厚7例,合并子宫肌瘤10例,合并轻-中等量腹水16例。结论:卵泡膜细胞瘤具有一定的MRI表现特征,结合ADC值有助于提高诊断准确率。Objective:To explore the magnetic resonance image(MRI)manifestations of ovarian thecoma.Methods:The MRI data of 26 cases of ovarian thecoma confirmed by clinical surgery and pathology were retrospectively analyzed,and the location,shape,boundary,signal,apparent diffusion coefficient(ADC)value measurement,enhancement method,presence or absence of combined pelvic effusion and endometrial changes were analyzed.Results:The 26 lesions were all single,with clear borders,and round-like masses.8 cases were solid,MRI plain scan T1WI showed equal or slightly low signal,T2WI showed equal or slightly high signal,T2WI lipid suppression sequence showed high signal;18 cases showed cystic and solid changes,and T2WI showed mixed hyperintensity.Diffusion weighted imaging of all cases showed slightly high signal,and the ADC average value was 1.380×10^(-3) mm^(2)/s;the enhanced scan showed progressive enhancement,but it was still lower than the myometrial signal.7 cases complicated with endometrial thickening;10 cases complicated with uterine fibroids;16 cases complicated with mild to moderate ascites.Conclusion:Ovarian thecoma has certain MRI characteristics,and the combination of ADC value can help improve the accuracy of diagnosis.
