摘　　要：目的研究二氯化锶治疗多发性骨转移瘤的临床效果。方法选取2016年1月至2019年9月经SPECT/CT联合其他影像检查及病理结果确诊为恶性肿瘤多发性骨转移瘤,并自愿使用二氯化锶进行治疗的66例多发性骨转移患者,观察治疗前、后患者骨痛缓解情况、生活质量、血常规指标、临床疗效及骨转移灶消退情况,并分析典型病例的影像学表现。结果66例患者治疗后随访2~18个月,平均9.5个月。治疗后,患者的VAS评分明显低于治疗前,KPS评分明显高于治疗前(P<0.05);治疗前、后,患者的WBC、PLT比较,差异无统计学意义(P>0.05)。治疗后1个月,重度疼痛、中度疼痛、轻度疼痛、无痛患者由治疗前的20、32、14、0例,改善至5、29、23、9例。治疗后1个月,患者的止痛有效率为77.27%,生活质量改善率为74.24%,治疗总有效率为84.85%。治疗≥3个月后复查骨扫描,骨转移灶治疗有效率为78.46%。结论二氯化锶治疗多发性骨转移瘤可缓解骨痛,降低骨转移灶及骨骼相关事件发生率,提高患者的生活质量,毒副作用小且疗效确切,值得在临床治疗中进一步借鉴。Objective To study the clinical effect of strontium dichloride in the treatment of multiple bone metastases.Methods A total of 66 patients with multiple bone metastases of malignant tumors diagnosed by SPECT/CT combined with other imaging and pathological examinations from January 2016 to September 2019 were selected,and all patients voluntarily received the treatment with strontium dichloride.The remission of bone pain,quality of life,blood routine indexes,clinical efficacy and regression of bone metastases were observed before and after treatment,and the imaging manifestations of typical cases were analyzed.Results Sixty-six patients were followed up for 2-18 months after treatment,with an average of 9.5 months.After treatment,the VAS score was significantly lower than that before treatment,while the KPS score was significantly higher than that before treatment(P<0.05);there were no significant differences in WBC and PLT before and after treatment(P>0.05).One month after treatment,patients with severe pain,moderate pain,mild pain and painlessness improved from 20,32,14 and 0 cases before treatment to 5,29,23 and 9 cases.One month after treatment,the effective rate of pain relief was 77.27%,the improvement rate of quality of life was 74.24%,and the total effective rate of treatment was 84.85%.Bone scan was reexamined after treatment for≥3 months,and the effective rate of treating the bone metastasis was 78.46%.Conclusion Strontium dichloride in the treatment of multiple bone metastases can alleviate bone pain,reduce the incidence of bone metastases and bone related events,and improve the quality of life of patients,with little toxic and side effects and definite curative effect,which is worthy of further reference in clinical treatment.
