比较超声引导下经直肠与经会阴途径前列腺穿刺活检术在前列腺癌诊断中的效果

摘　　要：目的比较超声引导下经直肠与经会阴途径前列腺穿刺活检术在前列腺癌诊断中的效果。方法选取我院2018年1月至2020年12月收治的120例前列腺特异抗原(PSA)升高患者作为研究对象,发放随机信封根据单双号将其分为观察组(超声引导下经会阴穿刺活检术)与对照组(超声引导下经直肠穿刺活检术),每组60例。比较两组的术中相关指标、对前列腺癌的检出情况及并发症发生情况。结果观察组的穿刺时间长于对照组,疼痛评分低于对照组,差异具有统计学意义(P<0.05)。观察组与对照组检出前列腺癌阳性例数分别为35、28例,分别占比58.33%、46.67%,差异无统计学意义(P>0.05)。观察组的发热、疼痛、出血发生率均低于对照组,差异具有统计学意义(P<0.05)。结论超声引导下经直肠与经会阴途径前列腺穿刺活检术均能够有效检出前列腺癌。超声引导下经会阴前列腺穿刺活检术具有穿刺时间长,疼痛程度轻,发热、出血发生率低的特点,临床应结合患者实际情况进行选择。Objective To compare the effects of ultrasound-guided transrectal and transperineal prostate biopsy in the diagnosis of prostate cancer.Methods A total of 120 patients with elevated prostate specific antigen(PSA)treated in our hospital from January 2018 to December 2020 were selected as the research objects.The random envelopes were issued,the patients were divided into observation group(ultrasound-guided transperineal prostate biopsy)and control group(ultrasound-guided transrectal prostate biopsy)according to single and double numbers,with 60 cases in each group.The intraoperative related indexes,the detection of prostate cancer and the occurrence of complications were compared between the two groups.Results The puncture time of the observation group was longer than that of the control group,and the pain score was lower than that of the control group,and the differences were statistically significant(P<0.05).The number of positive cases of prostate cancer in the observation group and the control group were 35 and 28 cases,accounting for 58.33%and 46.67%respectively,the difference was not statistically significant(P>0.05).The incidences of fever,pain and bleeding in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Ultrasound-guided transrectal and transperineal prostate biopsy can effectively detect prostate cancer.Ultrasound-guided transperineal prostate biopsy has the characteristics of long puncture time,light pain degree and low incidence of fever and bleeding.It should be selected according to the actual situation of patients.
