摘 要:目的:探讨晚期宫颈癌新辅助化疗(NACT)前后血清鳞状细胞癌抗原(SCC-Ag)、血管内皮生长因子(VEGF)和糖类抗原-125(CA125)水平变化及其与化疗敏感性的相关性分析。方法:选取某院收治的晚期宫颈癌患者153例为研究对象,患者均接受紫杉醇联合卡铂(TP)新辅助化疗方案,依据RECIST1.1版肿瘤疗效评价标准将其分为化疗敏感组(103例)和化疗不敏感组(50例),比较2组患者化疗前后血清SCC-Ag、VEGF、CA125水平变化。依据化疗后患者血清SCC-Ag、VEGF、CA125下降百分比分为<40%组、40%~60%组和>60%组,比较患者肿瘤直径变化情况及化疗反应率。分析血清SCC-Ag、VEGF、CA125下降百分比与TP方案化疗的敏感性。结果:1、2、3周期化疗后,血清SCC-Ag、VEGF、CA125水平均下降(P<0.05);化疗敏感组化疗前血清SCC-Ag、VEGF、CA125水平高于化疗不敏感组,化疗后低于化疗不敏感组(P<0.05);NACT有效率为67.32%(103/153);<40%、40%~60%和>60%3组所对应的肿瘤直径变化值与化疗反应率比较,差异均有统计学意义(P<0.05);Spearman相关性分析显示,晚期宫颈癌新辅助化疗后患者血清SCC-Ag、VEGF、CA125下降百分比与化疗敏感性呈正相关(P<0.05)。结论:晚期宫颈癌患者新辅助化疗后血清SCC-Ag、VEGF、CA125水平变化与其化疗敏感性呈正相关。NACT后SCC-Ag、VEGF、CA125水平下降幅度越大,肿瘤直径减少越多,化疗反应率及化疗敏感性越高。Objective:To analyze variations in serum squamous cell carcinoma antigen(SCC-Ag),vascular endothelial growth factor(VEGF)and glucose antigen-125(CA125)levels before and after neoadjuvant chemotherapy(NACT)for advanced cervical cancer and to investigate their correllations with chemosensitivity.Methods:A total of 153 patients with advanced cervical cancer admitted to a hospital were selected as research objects.All received paclitaxel with carboplatin(TP)neoadjuvant chemotherapy.According to the RECIST1.1 version of the tumor efficacy evaluation criteria,the patients were divided into a chemotherapy sensitivity group(n=103)and a chemotherapy insensitivity group(n=50),and the changes in serum SCC-Ag,VEGF and CA125 levels before and after chemotherapy were compared between the two groups.Based on the decline percentage of serum SCC-Ag,VEGF,CA125 after the chemotherapy,the patients were re-divided into three groups:<40%,40%~60%,and>60%,and the tumor diameter changes and chemotherapy response rate were compared.The percentage decrease in serum SCC-Ag,VEGF,CA125 in correlation with chemosensitivity of the TP regimen was analyzed.Result:Serum SCC-Ag,VEGF,CA125 levels all decreased after 1 cycle,2 cycles,and 3 cycles of the chemotherapy(P<0.05).Serum SCC-Ag,VEGF,CA125 levels before chemotherapy were higher in the chemosensitive group than in the insensitive group,whereas after the chemotherapy,the levels were lower than those in the chemo-insensitive group(P<0.05).The effective rate of NACT was 67.32%(103/153).In comparison of the tumor diameter change values with the chemotherapy response rate in the<40%,40%~60%,and>60%groups,the differences were all statistically significant(P<0.05).Spearman Correlation analysis suggested that percentage of decrease in SCC-Ag,VEGF,CA125 after chemotherapy was positively associated with chemosensitivity in patients with advanced cervical cancer(P<0.05).Conclusion:The changes in serum SCC-Ag,VEGF,CA125 levels before and after NACT for advanced cervical cancer were positively correlated w
