摘  要:目的分析血清微小RNA-330-5p(miR-330-5p)、miR-34a-5p、血管内皮生长因子(VEGF)水平与急性缺血性脑卒中患者脑侧支循环分级的相关性。方法选取2019年7月—2021年1月在石家庄市第三医院神经内科住院治疗且建立脑侧支循环的急性缺血性脑卒中患者104例为研究组,数字减影血管造影(DSA)检测脑侧支循环情况,根据开放层次分为一级侧支循环亚组(一级亚组)36例,二级侧支循环亚组(二级亚组)51例,三级侧支循环亚组(三级亚组)17例,同期选取医院健康体检者40例为健康对照组。检测血清miR-330-5p、miR-34a-5及VEGF水平,Pearson法分析急性缺血性脑卒中患者血清miR-330-5p、miR-34a-5p与VEGF水平的相关性,受试者工作特征曲线(ROC)评价血清miR-330-5p、miR-34a-5p、VEGF水平对急性缺血性脑卒中患者三级脑侧支循环建立的预测价值。结果研究组血清miR-330-5p、miR-34a-5p、VEGF水平均高于健康对照组(t/P=17.045/0.000、26.347/0.000、6.736/0.000);一级亚组、二级亚组、三级亚组血清miR-330-5p、miR-34a-5p、VEGF水平依次升高(F/P=54.260/0.000、161.507/0.000、11.309/0.000);Pearson分析结果显示,急性缺血性脑卒中患者血清miR-330-5p、miR-34a-5p与VEGF水平均呈正相关(r/P=0.674/0.000、0.624/0.000);ROC分析结果显示,血清miR-330-5p、miR-34a-5p、VEGF水平及三者联合预测急性缺血性脑卒中患者三级脑侧支循环建立的曲线下面积(AUC)分别为0.874、0.903、0.870、0.951,三者联合诊断效能更高。结论血清miR-330-5p、miR-34a-5p、VEGF水平随急性缺血性脑卒中患者脑侧支循环分级增加而升高,对三级脑侧支循环的建立有一定的预测价值。Objective To analyze the correlation between serum levels of microRNA-330-5p(miR-330-5p),miR-34a-5p,vascular endothelial growth factor(VEGF)and the grading of cerebral collateral circulation in patients with acute ischemic stroke.Methods From July 2019 to January 2021,104 patients with acute ischemic stroke who were hospitalized in the Department of Neurology of Shijiazhuang Third Hospital and established collateral circulation were selected as the research group.Digital subtraction angiography(DSA)was used to detect the brain According to the level of openness,the collateral circulation was divided into 36 cases in the first-level collateral circulation subgroup(first-level subgroup),51 cases in the second-level collateral circulation subgroup(secondary subgroup),and the third-level collateral circulation subgroup(Three-level subgroup)17 cases,and 40 cases of hospital health checkups were selected as the healthy control group during the same period.Serum miR-330-5p,miR-34a-5p and VEGF levels were detected,Pearson method was used to analyze the correlation between serum miR-330-5p,miR-34a-5p and VEGF levels in patients with acute ischemic stroke,receiver operating characteristic curve(ROC)was used to evaluate serum miR-330-5p,miR-34a-5p.The predictive value of VEGF levels in the establishment of tertiary cerebral collateral circulation in patients with acute ischemic stroke.Results The serum levels of miR-330-5p,miR-34a-5p,and VEGF in the study group were higher than those in the healthy control group(t/P=17.045/0.000,26.347/0.000,6.736/0.000).Serum levels of miR-330-5p,miR-34a-5p,and VEGF increased in the first,second,and third subgroups(F/P=54.260/0.000,161.507/0.000,11.309/0.000).Pearson analysis showed that serum miR-330-5p,miR-34a-5p and VEGF levels in patients with acute ischemic stroke were positively correlated(r/P=0.674/0.000,0.624/0.000).ROC analysis results showed that serum miR-330-5p,miR-34a-5p,VEGF levels and the three combined prediction area under the curve(AUC)for the establishment of tertiary ce
