颈部CTA联合ESSEN卒中风险评估量表对缺血性脑卒中的评价

摘　　要：目的探讨颈部CT血管造影(CTA)联合ESSEN卒中风险评估量表(ESRS)预测缺血性脑卒中风险的临床价值。方法将355例疑似缺血性脑卒中患者分为缺血性脑卒中组与非缺血性脑卒中组,比较两组患者颈动脉狭窄程度及ESRS分级。采用ROC曲线评价单独应用CTA、ESRS评分以及CTA联合ESRS评分对缺血性卒中的诊断价值。结果缺血性脑卒中组患者的ESRS评分[(3.53±1.88)分]高于非缺血性脑卒中组[(2.38±1.80)分](P<0.05)。缺血性脑卒中组患者符合阳性标准的比例(53.0%)高于非缺血性脑卒中组(30.4%)(P<0.05)。ESRS、CTA以及CTA联合ESRS诊断缺血性脑卒中的灵敏度分别为0.513、0.530、0.709,特异度分别为0.616、0.696、0.480。结论与单独使用ESRS或颈部CTA相比,颈部CTA联合ESRS可减少诊断缺血性脑卒中的漏诊率。Objective To explore the clinical value of cervical CT angiography(CTA)combined with ESSEN stroke risk score(ESRS)in predicting the risk of ischemic stroke.Methods 355 patients with suspected ischemic stroke were divided into ischemic stroke group and non-ischemic stroke group.The degree of carotid artery stenosis and ESRS grade were compared between the two groups.ROC curve was used to evaluate the value of CTA,ESRS score and CTA combined with ESRS score in the diagnosis of ischemic stroke.Results The ESRS score of ischemic stroke group(3.53±1.88)was higher than that of non-ischemic stroke group(2.38±1.80)(P<0.05).The proportion of patients who met the positive criteria in the ischemic stroke group(53.0%)was higher than that in the non-ischemic stroke group(30.4%)(P<0.05).The sensitivity of ESRS,CTA and CTA combined with ESRS in the diagnosis of ischemic stroke was 0.513,0.530 and 0.709,respectively,and the specificity was 0.616,0.696 and 0.480,respectively.Conclusion Compared with ESRS or cervical CTA alone,cervical CTA combined with ESRS can reduce the rate of missed diagnosis of ischemic stroke.
