摘 要:目的探讨和评估AccuContour软件在口腔颌面头颈肿瘤放疗患者口腔结构自动勾画的应用价值。方法选取我院在2020年5月至9月收治的30例口腔颌面头颈肿瘤放疗患者的定位CT图像,分别进行手工勾画和使用AccuContour软件进行自动勾画口腔结构。以手工勾画为标准,使用戴斯系数(Dice Similarity Coefficient,DSC)、体积差异百分比、质心距离差距和Hausdorff距离4类指标评估该软件的勾画准确性。结果自动和手工两种勾画方式勾画口腔结构的DSC为0.76±0.1,体积差异比为-49.43%~63.78%;在质心距离差异中,y(上下)方向和z(前后)方向的差异性大于x(左右)方向,三个方向的差异分别为(-0.02±0.09)cm、(-0.28±0.26)cm和(-0.31±0.55)cm。Hausdorff距离为(1.96±0.56)cm,DSC与体积差异比具有相关关系(P<0.05)。结论AccuContour软件在口腔颌面头颈肿瘤放疗患者的口腔结构自动勾画中具有较好的应用效果,但对于受金属伪影影响的CT定位图像勾画仍有所欠缺,需进一步手动修改。Objective To explore and evaluate the application value of AccuContour in automatic oral delineation in patient with oromaxillofacial head and neck tumors radiotherapy.Methods Thirty patients with oromaxillofacial head and neck cancer treated in our hospital from May to September 2020 were selected.The oral structure in CT images was manually and automatically by AccuContour.The accuracy of the software was evaluated using dice similarity coefficient(DSC),volume difference percentage,centroid distance gap,and hausdorff distance.Results The DSC of oral cavity was 0.76±0.1 and the volume difference ratio was-49.43%~63.78%.In the difference of centroid distance,the difference between y(up and down)direction and z(front and back)direction was greater than x(left and right)direction,the differences of three directions were(-0.02±0.09)cm,(-0.28±0.26)cm and(-0.31±0.55)cm,respectively.The Hausdorff distance was(1.96±0.56)cm.The DSC has a correlation with the volume difference ratio(P<0.05)and has a linear correlation.Conclusion AccuContour has a good application effect in the automatic delineation of oral structure in patients with oromaxillofacial head and neck tumor radiotherapy.But there is still a deficiency in the delineation of CT positioning image affected by metal artifacts,which requires further manual modification.
