宫颈癌同步放化疗中应用PEG-rhG-CSF 联合自拟扶正益气方剂临床观察

摘　　要：目的观察宫颈癌同步放化疗中应用聚乙二醇化重组人粒细胞刺激因子(PEG-rhG-CSF)联合自拟扶正益气方剂的临床效果。方法选取2018年2月-2020年10月于江西省妇幼保健院行根治性同步放化疗的80例宫颈癌患者,按照随机数字表法分为对照组与观察组,各40例。对照组于同步放化疗中应用PEG-rhG-CSF治疗,观察组在其基础上给予自拟扶正益气方剂治疗。比较两组临床疗效、不良反应、外周血象变化[白细胞计数(WBC)、血红蛋白(HGB)、血小板计数(PLT)]及生活质量(Kamofsky评分)。结果观察组治疗有效率高于对照组(P<0.05);观察组骨髓抑制、胃肠道反应、慢性放射性膀胱炎、慢性放射性肠炎、泌尿系统反应发生率均低于对照组(P<0.05);治疗后,两组外周血WBC、HGB、PLT指标均有下降,但观察组外周血WBC、HGB、PLT指标略高于对照组(P<0.05);治疗后,两组Kamofsky评分均高于治疗前,且观察组高于对照组(P<0.05)。结论宫颈癌同步放化疗中应用PEG-rhG-CSF联合自拟扶正益气方剂可增强其临床疗效,降低不良反应风险,缓解放化疗引起的外周血象变化,改善生活质量。Objective To observe the clinical effects of pegylated recombinant human granulocyte colony-stimulating factor(PEG-rhG-CSF)combined with self-made Fuzheng Yiqi prescription in concurrent radiotherapy and chemotherapy for cervical cancer.Methods Eighty patients with cervical cancer who underwent radical concurrent chemoradiotherapy in Jiangxi Maternal and Child Health Hospital from February 2018 to October 2020 were selected and divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was treated with PEG-rhG-CSF in concurrent chemoradiotherapy,and the observation group was treated with self-made Fuzheng Yiqi prescription on the basis of the control group.The clinical efficacy,adverse reactions,peripheral blood changes[white blood cell count(WBC),hemoglobin(HGB),platelet count(PLT)]and quality of life(Kamofsky score)were compared between the two groups.Results The effective rate of treatment in the observation group was higher than that in the control group(P<0.05).The incidences of bone marrow suppression,gastrointestinal reaction,chronic radiation cystitis,chronic radiation enteritis and urinary system reaction in the observation group were lower than those in the control group(P<0.05).After treatment,the peripheral blood WBC,HGB and PLT indexes of the two groups decreased,but the peripheral blood WBC,HGB and PLT indexes of the observation group were slightly higher than those of the control group(P<0.05).After treatment,the Kamofsky scores of the two groups were higher than those before treatment,and the observation group was higher than the control group(P<0.05).Conclusion The application of PEG-rhG-CSF combined with self-made Fuzheng Yiqi prescription in concurrent radiotherapy and chemotherapy for cervical cancer can enhance its clinical efficacy,reduce the risk of adverse reactions,relieve the changes in peripheral blood caused by radiotherapy and chemotherapy,and promote the improvement of quality of life.
