焦虑情绪与妊娠高血压妇女血压变异性、内皮素-1、一氧化氮的关系

摘　　要：目的探究焦虑情绪与妊娠高血压妇女血压变异性(BPV)、内皮素-1(ET-1)以及一氧化氮(NO)的关系。方法选取2018年4月~2019年8月于我院接受治疗的144例妊娠高血压妇女为研究对象,根据焦虑自评量表(SAS)评分将患者分为对照组、观察组1和观察组2。其中58例单纯妊娠高血压患者为对照组,49例妊娠高血压合并轻度焦虑患者为观察组1,37例妊娠高血压合并中重度焦虑患者为观察组2。比较三组患者24h收缩压标准差、24h舒张压标准差、昼间收缩压标准差、昼间舒张压标准差、夜间收缩压标准差以及夜间舒张压标准差等BPV指标;比较两组患者ET-1和NO水平;使用logistics多因素回归分析研究BPV各指标与ET-1、NO之间的关系。结果三组患者24h收缩压标准差、昼间收缩压标准差、夜间收缩压标准差以及夜间舒张压标准差存在差异,且观察组2以上指标较其他两组均处于更高水平(P<0.05)。三组患者的ET-1水平存在明显差异(58.34±16.78ng/L;76.98±20.51ng/L;95.30±18.77ng/L)、NO水平存在明显差异(81.22±6.84μmol/L;70.02±9.45μmol/L;64.32±5.19μmol/L)(P<0.05)。logistics多因素回归分析显示24h收缩压标准差、夜间收缩压标准差以及夜间舒张压标准差是影响ET-1和NO水平的相关因素。结论焦虑情绪能够影响妊娠高血压妇女的血压变异性,进一步影响其内皮素-1和一氧化氮含量的变化。Objective To explore the relationship among anxiety and blood pressure variability(BPV),endothelin-1(ET-1)and nitric oxide(NO)in pregnant women with hypertension(PIH).Methods 144 pregnant women with hypertension who were treated in our hospital from April 2018 to August 2019 were selected as the study objects.According to the SAS score,the patients were divided into control group,observation group 1 and observation group 2.58 cases of PIH were in the control group,49 cases of PIH with mild anxiety were in the observation group 1,37 cases of PIH with moderate or severe anxiety were in the observation group 2.BPV indexes were compared among the three groups;ET-1 and NO levels were compared among the three groups;the relationship between BPV indexes and ET-1 and NO was studied by logistic multi factor regression analysis.Results There were difference in the standard difference of 24h systolic pressure,the standard difference of systolic pressure among the three groups,and the index above in the observation group 2 was at a higher level(P<0.05).There were difference in level of NO(58.34±16.78ng/L;76.98±20.51ng/L;95.30±18.77ng/L)and ET-1(81.22±6.84μmol/L;70.02±9.45μmol/L;64.32±5.19μmol/L)among the three groups(P<0.05).Logistic regression analysis showed that 24-hour systolic blood pressure standard deviation,night systolic blood pressure standard deviation and night diastolic blood pressure standard deviation were the related factors affecting the level of ET-1 and NO.Conclusion Anxiety can affect the blood pressure.variability of pregnant women with hypertension,and further cause the changes of endothelin-1 and nitric oxide content.
