血清胃蛋白酶原与老年胃溃疡患者溃疡愈合质量的相关性

摘　　要：[目的]观察老年(≥60岁)胃溃疡患者不同溃疡愈合质量的血清胃蛋白酶原表达,分析血清胃蛋白酶原与溃疡愈合质量的相关性。[方法]回顾性分析78例老年胃溃疡患者病历资料,将其作为胃溃疡组;另选同期在医院常规体检证实为健康的老年人80例作为健康对照组。胃溃疡患者均视情况给予合理治疗,治疗8周后评价溃疡愈合质量。比较健康对照组、溃疡组血清胃蛋白酶原水平;分析血清胃蛋白酶原水平对溃疡愈合质量的影响,检验其预测溃疡愈合不良的价值。[结果]胃溃疡组入院时、治疗结束时的血清胃蛋白酶原各指标水平均高于健康对照组,差异有统计学意义(P<0.05);入院时、治疗结束时,愈合不良患者的血清胃蛋白酶原各指标水平均高于愈合良好患者,差异有统计学意义(P<0.05);作Logistic回归分析,结果显示,各血清胃蛋白酶原水平增高均是胃溃疡愈合不良的影响因素(OR>1,P<0.05);ROC曲线结果显示,胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ预测溃疡愈合不良的AUC分别为0.922、0.766,均有一定预测价值。[结论]老年胃溃疡患者入院时血清胃蛋白酶原增高可能预示着治疗后溃疡愈合质量不佳,尽早检测老年胃溃疡患者血清胃蛋白酶原水平,可以指导临床为胃溃疡患者制定更为合理的治疗计划,对提高溃疡愈合质量有一定临床意义。[Objective]To observe the expression of serum pepsinogen of different ulcer healing quality in senile patients with gastric ulcer,and to analyze the correlation between serum pepsinogen and ulcer healing quality.[Methods]The medical data of 78 senile patients with gastric ulcer who admitted in The First Hospital of Yulin City from November 2017 to November 2019 were retrospectively analyzed and taken as gastric ulcer group,and 80 healthy senile people who were confirmed to be healthy by routine physical examination in the hospital during the same period were selected as healthy control group.All patients with gastric ulcer were given reasonable treatment as appropriate,and the ulcer healing quality was evaluated after 8 weeks of treatment.The levels of serum pepsinogen in healthy control group and ulcer group were compared.The effect of serum pepsinogen levels on ulcer healing quality was analyzed,and its value in predicting poor ulcer healing was examined.[Results]The levels of serum pepsinogen in gastric ulcer group on admission and at the end of treatment were higher than those in healthy control group,and the differences were statistically significant(P<0.05).The levels of serum pepsinogen in poor healing group on admission and at the end of treatment were higher than those in good healing group,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that the overexpression of each serum pepsinogen level was the influencing factor of poor gastric ulcer healing(OR>1,P<0.05).The results of ROC curve showed that the area under the curve(AUC)of pepsinogenⅠand pepsinogenⅡin predicting poor ulcer healing was 0.922 and 0.766,respectively,which had certain predictive value.[Conclusion]High expression of serum pepsinogen in senile patients with gastric ulcer may indicate poor ulcer healing quality after treatment.Early detection of serum pepsinogen level in senile patients with gastric ulcer can provide the guidance for clinical development of more reasonable treatment plan in
