甲状腺乳头状癌组织中E-cadherin、ZEB1表达水平与临床病理特征和预后的关系

摘　　要：目的探讨甲状腺乳头状癌(PTC)组织中E-钙黏着蛋白(E-cadherin)、锌指结合蛋白1(ZEB1)表达水平与临床病理特征和预后的关系。方法选取该院2014年6月至2017年9月收治的68例接受PTC切除术的PTC患者,取PTC组织为病例组,同时取癌旁正常组织(距癌组织5 cm以上)为对照组。收集两组临床病理特征,采用RT-PCR及免疫组化法测定E-cadherin、ZEB1表达水平,采用多元Cox逐步回归分析探讨PTC患者的预后影响因素。结果病例组E-cadherin mRNA表达水平低于对照组,ZEB1 mRNA表达水平高于对照组(P<0.05)。病例组E-cadherin蛋白表达评分、蛋白表达阳性率低于对照组,ZEB1蛋白表达评分、蛋白表达阳性率高于对照组(P<0.05)。E-cadherin、ZEB1蛋白表达情况在不同肿瘤最大径、TNM分期、浸润深度、淋巴血管间隙浸润、淋巴结转移、肿瘤复发情况患者之间差异有统计学意义(P<0.05),且当肿瘤最大径≥5 cm、TNM分期越高、浸润深度越深、有淋巴血管间隙浸润、有淋巴结转移、肿瘤复发时,E-cadherin蛋白表达阳性率越低,ZEB1蛋白表达阳性率越高,而E-cadherin、ZEB1蛋白表达情况在<50岁和≥50岁患者之间差异无统计学意义(P>0.05)。E-cadherin阳性组、ZEB1阴性组3年生存率升高、生存期延长(P<0.05)。E-cadherin蛋白表达阳性(HR=0.54,95%CI:0.34~0.86)和ZEB1蛋白表达阴性(HR=0.61,95%CI:0.46~0.81)为PTC患者预后的独立保护因素(P<0.05)。结论PTC组织中E-cadherin表达水平降低、ZEB1表达水平升高;PTC临床病理特征对E-cadherin、ZEB1蛋白表达情况有影响;E-cadherin蛋白表达阳性、ZEB1蛋白表达阴性的PTC患者能获得较好的预后。Objective To investigate the expression of E-cadherin and zinc finger E-box-binding homeobox 1(ZEB1)in papillary thyroid carcinoma(PTC)tissue and its correlation with clinicopathological features and prognosis.Methods A total of 68 patients with PTC in the hospital from June 2014 to September 2017 were selected.PTC tissues were taken as the case group,and the normal tissues more than 5 cm away from the PTC tissues were taken as the control group.The clinicopathological features of the two groups were collected,and the expression levels of E-cadherin and ZEB1 were detected by RT-PCR and immunohistochemistry.Multivariate Cox regression analysis was used to explore the prognostic factors of PTC.Results The expression level of E-cadherin mRNA in the case group was lower than that in the control group,and the expression level of ZEB1 mRNA in the case group was higher than that in the control group(P<0.05).The expression score and positive rate of E-cadherin protein in the case group were lower than those in the control group,while the expression score and positive rate of ZEB1 protein in the case group were higher than those in the control group(P<0.05).The expression of E-cadherin protein was significantly affected by tumor maximum diameter,TNM stage,depth of invasion,lymph node metastasis and recurrence(P<0.05),and the positive rate of E-cadherin protein was lower and the positive rate of ZEB1 protein was higher in patients with tumor maximum diameter≥5 cm,higher TNM stage,deeper invasion depth,lymphatic vascular space infiltration,lymph node metastasis or recurrence,but whether the age was over 50 years old had no effect on the expression of E-cadherin and ZEB1 protein(P>0.05).The 3-year survival rate and survival time of E-cadherin positive group and ZEB1 negative group were significantly increased(P<0.05).Positive expression of E-cadherin protein(HR=0.54,95%CI:0.34-0.86)and negative expression of ZEB1 protein(HR=0.61,95%CI:0.46-0.81)were independent protective factors for the prognosis of patients with PTC(P<0.
