眼睑再造术治疗眼睑基底细胞癌的疗效及术后整合素αvβ3、核因子-κB65和E-cadherin的表达情况

摘　　要：目的探究眼睑再造术治疗眼睑基底细胞癌的疗效及术后整合素αvβ3、核因子-κB65(NF-κBP65)和E-cadherin的表达情况。方法将75例眼睑基底细胞癌患者按治疗方法不同分为对照组(n=35)和观察组(n=40),对照组患者实施标准化肿瘤切除术治疗,观察组在对照组基础上实施眼睑再造术,比较两组患者的视力情况,整合素αvβ3、NF-κBP65、E-cadherin阳性表达情况,治疗满意度和生活质量。结果术后2周,观察组患者视力情况优于对照组(P﹤0.05);两组患者肿瘤组织中整合素αvβ3、NF-κBP65和E-cadherin阳性表达率均高于癌旁组织,差异均有统计学意义(P﹤0.05)。不同肿瘤体积、脉管侵犯情况、脉管转移情况眼睑基底细胞癌患者肿瘤组织中整合素αvβ3、NF-κBP65和E-cadherin阳性表达情况比较,差异均有统计学意义(P﹤0.05)。观察组患者满意度明显高于对照组,差异有统计学意义(P﹤0.01)。术后,两组患者健康状况调查简表(SF-36)各维度评分均升高,且观察组均高于对照组(P﹤0.05)。结论眼睑再造术应用于眼睑基底细胞癌,可改善患者的视力水平,提高患者的治疗满意度和生活质量,整合素αvβ3、NF-κBP65和E-cadherin阳性表达可能与眼睑基底细胞癌发生发展有关。Objective To investigate the effect of eyelid reconstruction in the eyelid basal cell carcinoma and the expression of integrin αvβ3,nuclear factor-κB65(NF-κBP65)and E-cadherin after surgery.Method A total of 75 patients with eyelid basal cell carcinoma were divided into control group(n=35)and observation group(n=40)according to different treatment methods.The control group was treated with standardized tumor resection,while the observation group was treated with eyelid reconstruction based on the regimen taken by the control group.The visual acuity,positive rate of integrin αvβ3,NF-κBP65,E-cadherin,treatment satisfaction,and quality of life of the two groups were counted and compared.Result Two weeks after the operation,the visual acuity of the observation group was better than that of the control group(P<0.05).The positive rates of integrin αvβ3,NF-κBP65,and E-cadherin in tumor tissues from two groups were higher than those in adjacent tissues,and the differences were statistically significant(P<0.05).There were statistically significant differences in the expression of integrin αvβ3,NF-κBP65,and E-cadherin in tumor tissues from patients with different tumor volumes,and vascular invasion or metastasis(P<0.05).The satisfaction of patients in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.01).After the operation,the scores of each dimension of the MOS item-short from health survey(SF-36)of the two groups increased,and the observation group were higher than the control group(P<0.05).Conclusion The application of eyelid reconstruction in eyelid basal cell carcinoma could improve the visual acuity of patients,and improve their satisfaction and quality of life.The positive of integrinαvβ3,NF-κBP65,and E-cadherin may be related to the initial and development of eyelid basal cell carcinoma.
