摘 要:目的对比超微血管显像(SMI)、彩色多普勒血流显像(CDFI)在甲状腺癌与甲状腺良性肿块鉴别诊断中的价值。方法对200例甲状腺结节患者分别在SMI与CDFI模式下进行诊断并行病理检查,以病理检查结果为金标准,比较SMI与CDFI模式诊断甲状腺癌与甲状腺良性肿块的灵敏度、特异度、准确度、阳性预测值、阴性预测值,以及SMI、CDFI模式下甲状腺癌与甲状腺良性肿块的血流分级。结果病理检查结果显示,200例甲状腺结节患者中,87例诊断为甲状腺癌,113例诊断为甲状腺良性肿块。SMI诊断甲状腺癌的灵敏度为93.10%,特异度为91.15%,准确度为92.00%,阳性预测值为89.01%,阴性预测值为94.50%;CDFI诊断甲状腺癌的灵敏度为79.31%,特异度为70.80%,准确度为74.50%,阳性预测值为67.65%,阴性预测值为81.63%。SMI模式下甲状腺癌与甲状腺良性肿块血流检出率均高于CDFI模式(P﹤0.05)。结论SMI鉴别诊断甲状腺癌与甲状腺良性肿块的能力优于CDFI,具有较高的应用价值。Objective To compare the superb microvascular imaging(SMI)and color Doppler flow imaging(CDFI)in the differential diagnosis of thyroid carcinoma and benign thyroid masses.Method A total of 200 patients with thyroid nodules were diagnosed and pathologically examined in SMI and CDFI,respectively,and the results of pathological examination were taken as the golden standard.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SMI and CDFI in the diagnosis of thyroid carcinoma and benign thyroid masses were compared,and the blood flow grades of thyroid carcinoma and benign thyroid masses under SMI and CDFI were compared.Result Pathological examination showed that among the 200 patients with thyroid nodules,87 cases were diagnosed as thyroid cancer and 113 cases were diagnosed as benign thyroid masses.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SMI in the diagnosis of thyroid cancer were 93.10%,91.15%,92.00%,89.01% and 94.50%,respectively,and the CDFI were 79.31%,70.80%,74.50%,67.65% and 81.63%,respectively.The detection rate of blood flow in thyroid carcinoma and benign thyroid masses in SMI mode was significantly higher than that in CDFI mode(P<0.05).Conclusion SMI is superior to CDFI in differential diagnosis of thyroid carcinoma and benign thyroid masses,with higher application value.
