摘　　要：目的研究动态增强磁共振(DCE-MRI)对乳腺癌腋窝淋巴结转移(LNM)的诊断价值。方法收集96例乳腺癌患者的DCE-MRI资料,以患者腋窝淋巴结(LN)病理检查结果为金标准,将入选LN划分为转移组与非转移组,对比两组患者的临床特征及LN的DCE-MRI特征,分析各项特征及DCE-MRI与金标准间的一致性,计算各项诊断效能指标,采用受试者工作特征(ROC)曲线分析DCE-MRI各项特征及DCE-MRI对乳腺癌LNM的诊断价值。结果两组患者病理分型、激素受体情况比较,差异均无统计学意义(P﹥0.05)。转移组与非转移组LN的各DCE-MRI特征比较,差异均有统计学意义(P﹤0.01)。DCE-MRI诊断乳腺癌LNM的准确度为98.14%,灵敏度为98.84%,特异度为97.33%,与金标准间具有较高一致性(P﹤0.01)。DCE-MRI对乳腺癌LNM具有较高的诊断价值(P﹤0.05);各项特征中增强前后差值≥30诊断价值最高(P﹤0.05)。结论DCE-MRI对乳腺癌LNM具有较高诊断价值,可为乳腺癌LNM的诊断提供必要参考。Objective To investigate the diagnostic value of dynamic contrast enhancement-magnetic resonance imaging(DCE-MRI)in breast cancer with axillary lymph node metastasis(LMN).Method The DCE-MRI data of 96 breast cancer patients were collected,and the pathological examination results of the axillary lymph nodes(LN)of the patients were set up as the gold standard.The selected subjects were divided into the metastatic and non-metastatic groups,and the clinical features and the DCE-MRI features of LN of the two groups of patients were compared.The characteristics and the consistency between DCE-MRI and the gold standard were analyzed,and various diagnostic power indicators were also counted.The receiver operating characteristic(ROC)curves were drawn to analyze the characteristics of DCEMRI and evaluate the diagnosis of breast cancer LNM by DCE-MRI value.Result There were no statistically significant difference in pathological type and hormone receptor status between the two groups of patients(P>0.05).The DCEMRI characteristics of LN between the metastatic group and the non-metastatic group showed statistically significant differences(P<0.01).The accuracy of DCE-MRI in diagnosing breast cancer LNM was 98.14%,the sensitivity was 98.84%,and the specificity was 97.33%,which was highly comparable to the gold standard(P<0.01).Besides,DCE-MRI had a high diagnostic value for breast cancer LNM(P<0.05).Among the varying characteristics,the difference between before and after enhancement≥30 contained the highest diagnostic value(P<0.05).Conclusion DCE-MRI has a high value in the diagnosis of breast cancer’s axillary lymph node metastasis,which can provide a necessary reference for breast cancer diagnosis of LMN status.
