泪小管塞栓结合人工泪液、普拉洛芬滴眼液治疗中度及以上干眼患者主客观症状评分的影响

摘　　要：目的探讨泪小管塞栓结合人工泪液、普拉洛芬滴眼液治疗中度及以上干眼患者主客观症状评分的影响。方法根据电脑产生随机数法将该院2019年9月—2020年9月收治的中度及以上干眼症患者106例分为对照组(53例)与观察组(53例)。对照组给予人工泪液与普拉洛芬滴眼液治疗,观察组予以泪小管塞栓配合人工泪液和普拉洛芬滴眼液治疗。比较两组主观症状与客观指标、泪液炎症因子水平、疗效。结果治疗后,观察组眼痛、干涩、异物感、畏光及流泪症状评分较对照组低,差异有统计学意义(P<0.05)。观察组治疗后泪膜破裂时间(BUT)较对照组长,角膜荧光素染色(FL)评分、眼红指数较对照组低,泪液分泌实验较对照组佳,差异有统计学意义(P<0.05)。观察组治疗后白介素-18(IL-18)及白介素-1β(IL-1β)水平较对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率98.11%较对照组83.02%高,差异有统计学意义(χ^(2)=7.067,P=0.008)。结论在中度及以上干眼症患者治疗中泪小管塞栓与人工泪液、普拉洛芬滴眼液的联合运用,可减轻其主观症状评分,改善其客观指标,降低其泪液炎症因子水平,提升临床治疗效果。Objective To investigate the effect of lacrimal canalicular plug combined with artificial tears and pranoprofen eye drops on the subjective and objective symptom scores of patients with moderate or above dry eye.Methods According to the computer-generated random number method,106 patients with moderate or above dry eye who were admitted to the hospital from September 2019 to September 2020 were divided into a control group(53 cases)and an observation group(53 cases).The control group was treated with artificial tears and pranoprofen eye drops,and the observation group was treated with lacrimal canalicular plugs combined with artificial tears and pranoprofen eye drops.The subjective symptoms and objective indicators,levels of tear inflammatory factors,and curative effects were compared between the two groups.Results After treatment,the scores of eye pain,dryness,foreign body sensation,photophobia and tearing symptoms in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The tear film rupture time(BUT)of the observation group after treatment was longer than that of the control group,the corneal fluorescein staining(FL)score and jealous index were lower than those of the control group,and the tear secretion experiment was better than that of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-18(IL-18)and interleukin-1β(IL-1β)in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The total effective rate of treatment in the observation group 98.11%was higher than that in the control group(83.02%),the difference was statistically significant(χ^(2)=7.067,P=0.008).Conclusion In the treatment of moderate and above dry eye patients,the combined use of lacrimal canalicular plugs,artificial tears,and pranoprofen eye drops,it can reduce the subjective symptom score,improve the objective indicators,reduce the level of tear inflamm
